THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests removal of duplicate orders awarding him the Good Conduct Medal (1st Award) and (2nd Award) from his official military personnel file (OMPF).
2. The applicant states he was issued duplicate award orders on 25 August 2004 and 14 November 2007 for the Army Good Conduct Medal (1st Award) and (2nd Award), respectively.
3. The applicant provides Army Good Conduct Medal orders dated 25 August 2004 and 14 November 2007
CONSIDERATION OF EVIDENCE:
1. The applicant's military records show that he initially enlisted in the Regular Army on 24 May 2001, and continues to serve having reenlisted on 17 June 2005 and 4 December 2008. As of the date of his application to the Board, he was serving on active duty with the 1st Sustainment Command, Fort Bragg, North Carolina, in the rank of staff sergeant (SSG)/E-6.
2. The applicant's military records located on the integrated Personnel Electronic Records Management System (iPERMS) contain the following orders awarding him the Army Good Conduct Medal as indicated:
a. Headquarters, Tripler Army Medical Center, Permanent Order
Number 210-005, dated 28 July 2004 (1st Award for the period 24 May 2001 23 May 2004);
b. Headquarters, Tripler Army Medical Center, Permanent Order
Number 238-020, dated 25 August 2004 (1st Award for the period 24 May 2001 25 May 2004);
c. 62nd Medical Brigade, Permanent Order Number 254-07, dated
11 September 2007 (2nd Award for the period 24 May 2004 23 May 2007);
d. 62nd Medical Brigade, Permanent Order Number 318-03, dated
14 November 2007 (2nd Award for the period 24 May 2004 23 May 2007);
3. Army Regulation 600-8-104 (Military Personnel Information Management/
Records) provides policy and procedures for maintenance of a Soldier's personal information. Paragraph 2-4a states that once placed in the OMPF, a document becomes a permanent part of that file. The document will not be removed from an OMPF or moved to another part of the OMPF unless directed by, among other agencies, the Army Board for Correction of Military Records or the OMPF custodian when documents have been improperly filed.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record shows the applicant was awarded the Army Good Conduct Medal (1st Award) for the period 24 May 2001 to 23 May 2004 under Permanent Orders 210-005, dated 28 July 2004. His OMPF also contains Permanent Order 238-020, dated 25 August 2004, again awarding him the Army Good Conduct Medal (1st Award) for nearly the same period from 24 May 2001 to 25 May 2004, extending this period by two days. As a result, the period for which this award is announced is incorrectly shown on this duplicate order.
2. The evidence of record also shows the applicant was awarded the Army Good Conduct Medal (2nd Award) for the period 24 May 2004 to 23 May 2007 under Permanent Orders 254-07, dated 11 September 2007. His OMPF also contains Permanent Order 318-03, dated 14 November 2007, again awarding him the Army Good Conduct Medal (1st Award) for the same period of service.
3. In view of the circumstances in this case, the applicant is entitled to have Headquarters, Tripler Army Medical Center, Permanent Order Number 238-020, dated 25 August 2004 and 62nd Medical Brigade, Permanent Order Number 318-03, dated 14 November 2007 revoked and removed from his OMPF. In addition, by ensuring the revocation orders are not maintained in the applicants OMPF.
4. Based on the foregoing, the applicants records should be corrected as recommended below.

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. revoking and removing Headquarters, Tripler Army Medical Center, Permanent Order Number 238-020, dated 25 August 2004 and 62nd Medical Brigade, Permanent Order Number 318-03, dated 14 November 2007; and
b. ensuring the orders revoking the duplicate orders are not filed in his OMPF.
