THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, the removal of two erroneously filed documents from his Official Military Personnel File (OMPF). 2. The applicant states: a. Permanent Order 237-008 (Army Achievement Medal (AAM)) was erroneously submitted by his unit's personnel section and was uploaded into the interactive Personnel Electronic Records Management System (iPERMS). He states his name is misspelled on the AAM and he believes the S1 subsequently submitted a corrected AAM, but failed to revoke the misspelled AAM. b. In the interest of justice, the Board should consider removing the duplicate award from his OMPF. His year group is scheduled to go before an Officer Separation Board from 22 through 25 September 2015. He discovered the duplicate award when his "My Board File" became available. 3. The applicant provides Permanent Order 237-008. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army (RA) on 13 January 2000. 2. On 5 August 2009, he was discharged for the purpose of accepting a commission in the U.S. Army. 3. On 6 August 2009, he was appointed as a second lieutenant in the RA. 4. The applicant provides and his record contains Permanent Order 237-008, issued by Headquarters, 34th Support Group, Yongsan Garrison, Korea, dated 24 August 2004, which award him the AAM for outstanding achievement during the Ulchi Focus Lens 04 from 22 August to 5 September 2004. The document misspells the applicant's name. 5. His OMPF contains Permanent Order Number 222-01, issued by Headquarters, Joint Multinational Readiness Center, dated 10 August 2015, which revokes Permanent Order 237-08. 6. His OMPF contains Permanent Order Number 246-005, issued by Headquarters, 34th Support Group, Yongsan, Korea, dated 2 September 2004, which award him the AAM for outstanding achievement during the Ulchi Focus Lens 04 from 22 August to 5 September 2004. 7. Army Regulation 600-8-104 (Military Personnel Information Management/Records) prescribes the policies and mandated operating tasks for the Military Personnel Information Management/Records Program of the Military Personnel System. It states that, once placed in the OMPF, a document becomes a permanent part of that file. The document will not be removed from a fiche or moved to another part of the fiche unless directed by the Army Board for Correction of Military Records (ABCMR). DISCUSSION AND CONCLUSIONS: 1. The evidence of record confirms the applicant has an order in his OMPF which awards him the AAM for outstanding achievement during the Ulchi Focus Lens 04 from 22 August to 5 September 2004; however, his name is misspelled. The order was subsequently revoked by Permanent Order Number 222-01, which also is resident in his OMPF. 2. His OMPF also contains Permanent Order Number 246-005, which correctly spells his name and awards him the AAM for outstanding achievement during the Ulchi Focus Lens 04 from 22 August to 5 September 2004. 3. In view of the foregoing, there would appear to be no harm in removing Permanent Order 237-008 and Permanent Order Number 222-01 from the performance folder of his OMPF.
