APPLICANT REQUESTS THAT:
1. Her Officer Selection Brief (OSB) for the CY00A, CY01B, and the CY02B
Lieutenant Colonel Selection Boards be corrected to reflect her correct
Duty Air Force Specialty Code (DAFSC).
2. Her Officer Selection Brief (OSB) for the CY02B Lieutenant Colonel
Selection Board be corrected to reflect her correct duty title and that she
receive Special Selection Board (SSB) consideration for promotion to the
grade of lieutenant colonel for the Calendar Year CY00A, CY01B, and the
CY02B Selection Boards.

APPLICANT CONTENDS THAT:
Her OSB that met the promotion boards in 2000, 2001, and 2002 did not
contain the “M” prefix on her AFSC which indicates “board certified.” She
believes the omission sent the wrong message to the board members.
Numerous attempts to add the prefix have been unsuccessful. She also
discovered a duty title (effective 4 February 2001) demonstrating increased
depth and breadth of responsibility may have been dropped from the duty
history in the correction process.
In support of her appeal, the applicant provided a personal statement,
information regarding the National Certification/Board Certification and
“M” Prefix for Nurse Corps Officers, Officer Selection Brief prepared for
the CY01B board, Officer Pre-Selection Brief dated 23 July 2002, Officer
Surf dated 2 September 2002, and other documentation.

STATEMENT OF FACTS:
The applicant is currently serving on extended active duty in the grade of
major effective and with a date of rank (DOR) of 1 June 1996.
Applicant was considered and not selected for promotion to the grade of
lieutenant colonel by the CY00A (28 November 2000), CY01B (5 November
2001), and the CY02B (12 November 2002) Central Lieutenant Colonel
Selection Boards.
The applicant’s OSR for the CY00A, CY01B, and CY02B board did not include
her Certification of Nurse Anesthetists.
The applicant’s OSB for the CY01B board did not reflect “YES” in the board
certified block. The CY02B OSB showed the “M” prefix on her “Duty” AFSC in
the assignment history block for the following effective dates: 21 October
1999, 27 April 2000, and 2 March 2002. Her duty title effective 4 February
2001 “Ambulatory Procedures/Nurse” was omitted from her CY02B OSB.

AIR FORCE EVALUATION:
AFPC/DPAMF2 recommended approval of the duty title correction. They
indicated that the applicant made numerous attempts to have her AFSC
updated with the “M” prefix. Per AFI 36-2105, dated 31 October 2002, “Do
not affix the “M” prefix to the “Duty” AFSC of officers;” however the
prefix is attached to the “Primary” AFSC to show board certification. The
applicant submitted an AF Form 2096, dated 27 September 2001, indicating
the “M” prefix should have been updated to her “Primary” AFSC effective
12 November 1999. Applicant’s OSB for the CY02B board dated 4 November
2002 incorrectly shows the “M” prefix on her “Duty” AFSC under “Assignment
History.” Further, applicant submitted copies of her Officer Preselection
Brief (OPB) dated 23 July 2003 for the CY02B board which indicates a duty
title on 4 February 2001 as “Ambulatory Procedures/Nurse Anesthetist Chief”
which was not shown on her OSB when meeting the CY02B board. Applicant
also submitted an OSB from the CY01B board that also shows the duty title
of “Ambulatory Procedures/Nurse Anesthetist Chief.” The duty title was not
on the applicant’s OSB when she met the CY02B board. Applicant’s current
record accurately reflects the “M” prefix on her “Primary” AFSC. The
applicant’s duty title is missing from her Duty History.

AFPC/DPPPO recommended partial approval. Based on AFPC/DPAMF2’s advisory
and the evidence provided, they recommend SSB consideration by the CY00B
board with inclusion of her board certificate; consideration by the CY01B
board with a corrected OSB to reflect “Yes” in the board certified block,
and inclusion of her board certificate; and consideration by the CY02B
board with inclusion of the 4 February 2001 duty title on the OSB and her
board certificate. They recommend denial for SSB consideration to include
the prefix “M” to the “Duty” AFSC on her OSBs.
They indicated that according to AFPC/DPAMF2’s advisory, the “M” prefix is
not affixed to the “Duty” AFSC of officers; however, it is attached to the
“Primary” AFSC to show board certification, which is subsequently reflected
on OSBs with a “YES” in the board certified block. They have verified that
the officer’s CY02B OSB incorrectly shows the “M” prefix on her “Duty” AFSC
in the assignment history block. As a result, the “M” prefix has been
deleted from her “Duty” AFSC in the Military Personnel Data System
(MILPDS).
Since the applicant was board certified when she met the CY01B board, as
evident by the “M” prefix attached to the officer’s “Primary” AFSC, her
CY01B OSB should have reflected “YES” in the board certified block.
The applicant contends her duty title, effective 4 February 2001, as
“Ambulatory Procedures/Nurse Anesthetist Chief,” was omitted from her CY02B
OSB. It appears that in the process of attempting to correct her “Duty”
AFSC, her 4 February 2001 duty title entry was erroneously deleted from
MILPDS and subsequently from her CY02B OSB. Since it was not the officer’s
fault that this duty title was erroneously omitted from her CY02B OSB when
she was attempting to correct other errors, SSB consideration is warranted.
Concerning the wallet-size certificate she contends was filed in her OSR,
they reviewed her OSR and did not find a small, or larger certificate of
her national certification. They contacted HQ AFPC/DPAMN and they stated
that they had received, certified and taken her certificate to HQ
AFPC/DPPBR1 on 17 November 2000, for filing in her OSR. Additionally, on 3
August 2001, they received her recertification, validated it, and took it
to AFPC/DPPBR1, for filing in her OSR. For unknown reasons, the
certificates were not filed in her OSR. On 28 June 2003, they provided
AFPC/DPAMN with a copy of the certificate the officer provided with her
application. AFPC/DPAMN validated the certificate and filed it in her OSR.
The officer demonstrated diligence in maintaining her record; therefore,
SSB consideration is warranted.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
On 18 July 2003, copies of the Air Force evaluations were forwarded to the
applicant for review and response within 30 days. As of this date, no
response has been received by this office.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate the
existence of an error or injustice to warrant providing the applicant
consideration by a Special Selection Board for the Calendar Years CY00A,
CY01B, and the CY02B Central Lieutenant Colonel Selection Boards. Based on
the evidence of record, the applicant has established that numerous errors
in her records were reviewed by the selection boards in question. While it
cannot be conclusively determined whether or not these errors were the
reason for her nonselections, the Board believes that it served to deprive
her of fair and equitable consideration. Therefore, the Board recommends
her record be corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force relating
to APPLICANT, to include the corrected Officer Selection Briefs (OSBs)
prepared for the Calendar Years 2000A, 2001B, and 2002B Central Lieutenant
Colonel Selection Boards, be considered for promotion to the grade of
lieutenant colonel by Special Selection Board for the Calendar Years 2000A,
2001B, and the 2002B Central Lieutenant Colonel Selection Boards.
