APPLICANT REQUESTS THAT:
Her assignment history on her Officer Selection Brief (OSB) be
corrected to reflect her correct duty titles and dates, and the award
of the M prefix be added to her Duty AFSC, and she be considered for
promotion to the grade of colonel by Special Selection Board (SSB).

APPLICANT CONTENDS THAT:
Concerning her M prefix, PC3 data entry problems during recent change
to MILMOD are well known. System issues were further complicated by
excessive down time due to severe weather and local outages. The
missing prefix was discussed with personnel technicians, squadron
commander and group commander without resolution. As to her Masters
Degree in Anesthesia, she requested that AFIT update information; this
was not accomplished by AFIT/RRC. The dates on her duty titles were
inappropriately changed; she was dual-hatted as 81 AMDS Senior Nurse
Executive while Deputy AMDS from May 2002 until she PCSed - December
2002. She was unable to view a selection brief after September 2002
due to PC3 problems at Keesler. MDG/CC was at the October review
board and title and dates were changed. She had reviewed her brief in
September.
She was also Senior Nurse Executive AMDS while Deputy AMDS from May
2002 to December 2002. She was unable to view her selection brief
after September 2002 due to PC3 problems noted above (and three
hurricanes).
In support of the appeal, applicant submits source documentation of M
prefix, an e-mail supporting inability to update, a copy of the DPAMN
homepage posting 14 April 2003. She was board certified in 1974 and
was never informed to send a copy to HQ AFPC/DPMAF until she saw this
posting. She always got her incentive special pay (ISP) for her
anesthesia certification. She also provided an e-mail from the USAF
Nurse Anesthesia Consultant, supporting her assertions concerning
AFIT/RRC’s failure to update the information about her Masters in
Anesthesia and copies of her OPRs showing correct dates and duty
titles.

STATEMENT OF FACTS:
The applicant is currently serving on extended active duty in the
grade of lieutenant colonel.
Applicant was considered and not selected for promotion to the grade
of colonel by the CY02B (3 December 2002) Colonel Central Selection
Board.
Applicant’s Officer Performance Reports (OPRs) from 1991 through 2002
reflect meets standards on all performance factors.

AIR FORCE EVALUATION:
AFPC/DPAMM states that a review of the applicant’s duty title shows
her duty titles dated 3 June 1999 to 3 September 2002 are reflected
exactly as they are on OPRs provided for reference. Applicant’s DAFSC
history is incorrect for entries 9 December 1992 to 21 February 1995
and 5 August 1999 to 2 May 2002 and must be correct at the base level
MPF Employment Section. They further state that the DAFSC correction
in accordance with (IAW) AFMAN 36-2105, attachment 3 states that M
prefix can not be awarded to DAFSCs only primary, second and third.
Additionally the M and T suffix does not exist for award to DAFSCs
46M3 and 46A3. Since they are unable to correct at headquarters
level, they recommend the Military Personnel Flight (MPF) Employment’s
section review the AFIs provided and correct the DAFSCs currently in
applicant’s duty history. Therefore, they recommend disapproval of
applicant’s request because adding this is against the AFI.

AFPC/DPPPO states that they agree that the applicant’s Master’s Degree
in Anesthesia should have been reflected on her OSB. Since she
exercised reasonable diligence to correct this error prior to
convening of the board, and AFIT/RRC admits that it was their error
that her data was not updated on time, SSB consideration is
warranted. Therefore, they recommend approval for SSB consideration
to include her Master’s Degree in Anesthesia on her CY02B OSB.
However, they recommend denial to include the prefix M to the Duty
AFSC on her OSB. They also recommend denial to meet an SSB with a
corrected duty history on her OSB since she provides no evidence to
support what she did to correct her record prior to the board
convening date.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
On 31 October 2003, copies of the Air Force evaluations were forwarded
to the applicant for review and response within 30 days. As of this
date, this office has received no response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate
the existence of probable error or injustice. In this respect, we
note that the Air Force agrees that the applicant’s DAFSC history is
incorrect for entries 9 December 1992 to 21 February 1995 and 5 August
1999 to 2 May 2002. They also agree that the applicant’s Master’s
Degree in Anesthesia should have been reflected on her OSB, and that
she be considered for promotion to the grade of colonel by Special
Selection Board for the CY02B selection board.
4. Insufficient relevant evidence has been presented to demonstrate
the existence of probable error or injustice. Applicant’s request to
have the M prefix added to her Duty AFSC was considered; however,
favorable action on her request is not recommended. We note that the
M prefix is not affixed to the Duty AFSC of officers; however, it is
attached to the primary, second, or third AFSC to show board
certification, which is subsequently reflected on OSBs with a “Yes” in
the board certified block.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force relating
to APPLICANT, be corrected to show that the Officer Selection Brief (OSB)
prepared for the Calendar Year 2002B Colonel Central Selection Board be
amended as follows:
a. Delete the 05 August 1999 entry under Assignment History.
b. Her duty title effective 3 June 1999 was “Deputy Chief,
Medical Staff.”
c. Her duty title effective 2 May 2002 was “Deputy Squadron
Commander.”
d. The entry “LVL: MAS, SPECIALTY/SCHOOL: Anesthesia, YR:
2002” be included in the Academic Education section.
It is further recommended that she be considered for promotion to the
grade of colonel by Special Selection Board for the Calendar Year 2002B
Colonel Central Selection Board with the above corrected Officer Selection
Brief as a matter of record.

Having received and considered the recommendation of the Air
Force Board for Correction of Military Records and under the
authority of Section 1552, Title 10, United States Code (70A Stat
116), it is directed that:
The pertinent military records of the Department of the Air
Force relating to, be corrected to show that the Officer Selection
Brief (OSB) prepared for the Calendar Year 2002B Colonel Central
Selection Board be amended as follows:
a. Delete the 05 August 1999 entry under Assignment
History.
b. Her duty title effective 3 June 1999 was “Deputy
Chief, Medical Staff.”
