APPLICANT REQUESTS THAT:

1.    Her Officer Selection Record (OSR)  prepared  for  the  Calendar  Year
2002B (CY02B) Lieutenant Colonel Selection Board  be  corrected  to  include
her Nurse Anesthesia recertification.

2.    Her Officer Selection Brief (OSB) for  the  CY02B  Lieutenant  Colonel
Selection  Board  be  corrected  to  reflect  her  correct  Duty  Air  Force
Specialty Code (DAFSC) and that she receive Special  Selection  Board  (SSB)
consideration for promotion to the  grade  of  lieutenant  colonel  for  the
CY02B Selection Board.

APPLICANT CONTENDS THAT:

Pertinent  documentation  was  not  included  in  her  OSR  for  the   CY02B
lieutenant colonel selection board.  Her  Nurse  Anesthesia  recertification
was not in her records and her DAFSC incorrectly included an “M” prefix.

In support of her appeal, the applicant provided her  Officer  Pre-Selection
Brief, dated 23 July 2002 and 5 September 2002 and other documentation.

Applicant’s complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:

The applicant is currently serving on extended active duty in the  grade  of
major effective and with a date of rank (DOR) of 1 May 1996.




Applicant was considered and not selected for  promotion  to  the  grade  of
lieutenant colonel by the  CY00A  (28  November  2000),  CY01B  (5  November
2001),  and  the  CY02B  (12  November  2002)  Central  Lieutenant   Colonel
Selection Boards.

The applicant’s OSB for the CY02B board  reflected  an  “M”  prefix  on  her
DAFSC under “Assignment History.”

The applicant’s OSR before  the  CY02B  lieutenant  colonel  board  did  not
contain the paper copy of her Nurse  Anesthesia  recertification.   However,
the OSB for the CY02B did reflect a  “YES”  entry  in  the  board  certified
block.

AIR FORCE EVALUATION:

AFPC/DPAMF2 had no recommendation.  They indicated that  the  applicant  was
certified in Nurse Anesthesia when the CY02B lieutenant colonel  board  met.
Applicant’s “M” prefix was present on her DAFSC incorrectly and should  have
been attached to her Primary AFSC.  Per AFI 36-2105, dated 31 October  2002,
“Do not affix the “M” prefix to the “Duty” AFSC of officers;”  however,  the
prefix is attached to the “Primary” AFSC to show board certification.”

The evaluation is at Exhibit C.

AFPC/DPPPO recommended denial.  They indicated that based  on  the  evidence
provided, and the advisory from AFPC/DPAMF2, they recommend denial  for  SSB
consideration.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

On 8 August 2003, copies of the Air Force evaluations were forwarded to  the
applicant for review and response within 30  days.   As  of  this  date,  no
response has been received by this office.

THE BOARD CONCLUDES THAT:

1.    The applicant has exhausted all remedies provided by existing  law  or
regulations.

2.    The application was timely filed.

3.    Sufficient relevant evidence has been  presented  to  demonstrate  the
existence of an error  or  injustice  to  warrant  providing  the  applicant
consideration by a Special Selection  Board  for  the  Calendar  Year  CY02B
Central Lieutenant Colonel  Selection  Board.   Based  on  the  evidence  of
record, the applicant has established that numerous errors  in  her  records
were reviewed by the selection  board  in  question.   While  it  cannot  be
conclusively determined whether or not these errors were the reason for  her
nonselection, the Board believes that it served to deprive her of  fair  and
equitable consideration.  Therefore, the  Board  recommends  her  record  be
corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air  Force  relating
to APPLICANT, be corrected to show that:

a.  The  Field  Grade  Officer  Performance  Report  (OPR),  AF  Form  707A,
rendered for the period 7 May 2002 through 13 December 2002, be  amended  in
Section 1, Item 4, Duty Air Force Specialty Code (DAFSC) to read 46M3,  vice
M46M3.

b.  The Officer Selection Brief (OSB) prepared for the Calendar  Year  2002B
Central Lieutenant Colonel Selection Board be corrected  in  the  Assignment
History Section by removing the “M” prefix  from  the  DAFSCs  effective  20
January 1998, 29 December 1998, 22 January 2000, and  6  May  2002,  thereby
changing the DAFSCs on each of those dates to read 46M3.

c.  The Officer Selection Record (OSR) prepared for the Calendar Year  2002B
Central Lieutenant Colonel Selection  Board  include  her  Nurse  Anesthesia
recertification, with an expiration date of 31 July 2004.

It is further recommended  that  the  corrected  record  be  considered  for
promotion to the grade of lieutenant colonel by a  Special  Selection  Board
for the Calendar Year 2002B Central Lieutenant Colonel Selection Board.

MEMORANDUM FOR THE CHIEF OF STAFF

      Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of Section
1552, Title 10, United States Code (70A Stat 116), it is directed that:

      The pertinent military records of the Department of the Air Force
relating to   , be corrected to show that:

        a.  The Field Grade Officer Performance Report (OPR), AF Form 707A,
rendered for the period 7 May 2002 through 13 December 2002, be amended in
Section 1, Item 4, Duty Air Force Specialty Code (DAFSC) to read 46M3, vice
M46M3.

        b.  The Officer Selection Brief (OSB) prepared for the Calendar
Year 2002B Central Lieutenant Colonel Selection Board be corrected in the
Assignment History Section by removing the “M” prefix from the DAFSCs
effective 20 January 1998, 29 December 1998, 22 January 2000, and 6 May
2002, thereby changing the DAFSCs on each of those dates to read 46M3.

        c.  The Officer Selection Record (OSR) prepared for the Calendar
Year 2002B Central Lieutenant Colonel Selection Board include her Nurse
Anesthesia recertification, with an expiration date of 31 July 2004.

        It is further directed that the corrected record be considered for
promotion to the grade of lieutenant colonel by a Special Selection Board
for the Calendar Year 2002B Central Lieutenant Colonel Selection Board.
