THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he was awarded the North Atlantic Treaty Organization (NATO) Medal as well as the Naval Reserve Meritorious Service Medal (2nd Award).

2.  The applicant states the above awards were not included on his DD Form 214 because he did not have the corroborating paperwork at the time.

3.  The applicant provides a NATO Medal Certificate and page 3 of his Navy Personnel 1070/604 (Navy Enlisted Qualifications History).

CONSIDERATION OF EVIDENCE:

1.  After a period of service in the U.S. Navy and the U.S. Navy Reserve, the applicant was ordered to active duty as a member of the U.S. Army Reserve on or about 23 August 2013 in support of Operation Enduring Freedom (OEF).

2.  On 16 May 2014, he was honorably released from active duty.  His DD Form 214 for this period shows he served in Afghanistan from 31 August 2013 through 19 April 2014.

3.  His DD Form 214 for the period ending 16 May 2014 also shows he was awarded or authorized the:

* Afghanistan Campaign Medal with campaign star
* Iraq Campaign Medal with four campaign stars
* Sea Service Deployment Ribbon
* Army Commendation Medal (4th Award)
* Joint Meritorious Unit Award
* U.S. Navy "E" Ribbon (2nd Award)
* U.S. Navy Good Conduct Medal
* Army Reserve Components Achievement Medal (4th Award)
* National Defense Service Medal with Bronze Service Star
* Armed Forces Expeditionary Medal
* Southwest Asia Service Medal with Bronze Service Star
* Global War on Terrorism Service Medal
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Overseas Service Ribbon (2nd Award)
* U.S. Navy and U.S. Marine Corps Overseas Service Ribbon (2nd Award)
* Armed Forces Reserve Medal with M Device
* U.S. Navy Sea Service Deployment Ribbon
* Driver and Mechanic Badge (for wheeled vehicles)

4.  The applicant provides a NATO Medal Certificate showing he was awarded the NATO Medal for service with NATO on operations in relation to the International Security Assistance Force (ISAF) Operation during the period 2 November 2012 to 6 November 2013.

5.  He also provides a copy of page 3 from his Naval Enlisted Qualifications History, annotating award of the Naval Reserve Meritorious Service Medal (2nd), for service from 3 June 2000 through 2 June 2003.

6.  Army Regulation 600-8-22 (Military Awards) prescribes policy and procedural guidance relating to the processing of awards and decorations.  It states the NATO Medal is awarded by the Secretary-General of NATO members of the Armed Forces of the United States who participate in designated NATO operations.  The Secretary of Defense approved acceptance of the NATO Medal for U.S. military personnel who served in the ISAF in Afghanistan (1 June 2003 to a date to be determined).

7.  Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transitional management, to include separation document preparation.  It states the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty.  It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty at the time of release from active duty, retirement, or discharge.  It states to list all federally recognized awards and decorations for all periods of service.

8.  Office of the Secretary of the Navy Instruction 1650.1G, Navy and Marine Corps Awards Manual, dated 7 January 2002, provides information and regulatory guidance on awards for individuals and units in the Naval Service.  The Naval Reserve Meritorious Service Medal is intended to provide Naval reservists an award equivalent to the Good Conduct Medal.  Subsequent to 1 January 1996, the period of eligibility for the award is 3 years for enlisted Naval Reservists who meet the minimum requirements delineated in this instruction manual.

DISCUSSION AND CONCLUSIONS:

1.  The available evidence supports the applicant's request for correction of his DD Form 214 to show the NATO Medal and the Naval Reserve Meritorious Service Medal (2nd Award).

2.  The NATO Medal Certificate he provides confirms he received this award for service in support of ISAF operations in Afghanistan.  Therefore, he is entitled to correction of his DD Form 214 to show this award.

3.  Page 3 from his Naval Enlisted Qualifications History confirms he was awarded the Naval Reserve Meritorious Service Medal (2nd Award) for service from 3 June 2000 through 2 June 2003.  Therefore, he is entitled to correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the NATO Medal and the Naval Reserve Meritorious Service Medal (2nd Award) to his DD Form 214 for the period ending 16 May 2014.
