THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to update his missing awards.  

2.  The applicant states, in effect, he did not have any of his awards at the time of his demobilization.  

3.  The applicant provides the following as evidence in support of his request:

* DD Form 214, period ending 16 August 2014
* Bronze Star Medal (BSM) Certificate, dated 15 May 2014
* Army Achievement Medal (AAM) Certificate, dated 11 June 2014
* AAM Certificate, dated 30 August 2009
* AAM Certificate, dated 25 June 2008
* North Atlantic Treaty Organization (NATO) Medal Certificate, for service with NATO as part of the International Security Assistance Force (ISAF) Operation, during the period 16 October 2013 to 16 June 2014

CONSIDERATION OF EVIDENCE:

1.  The applicant, having prior enlisted service, was appointed as a Reserve commissioned officer of the Army on 4 August 2011.  He was commissioned as an 88A (Transportation Officer).  

2.  The applicant was mobilized and ordered to active duty in support of Operation Enduring Freedom.  He entered active duty on 30 August 2013, completed 11 months and 17 days of net active service, and was honorably released from active duty on 16 August 2014 by reason of completion of required active service.  His DD Form 214 shows in:

* Block 12f (Foreign Service), 0 year(s), 8 months, 23 days
* Block 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), the: 

* Army Commendation Medal
* AAM
* Afghanistan Campaign Medal with Campaign Star
* Iraq Campaign Medal with two Campaign Stars
* Global War on Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* National Defense Service Medal
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Overseas Service Ribbon (2nd Award)
* Armed Forces Reserve Medal with M Device

* Block 18 (Remarks),

3.  His record contains a DA Form 638 (Recommendation for Award), annotated as Permanent Orders Number 177-04, dated 22 June 2008, which awarded him an AAM for achievement for the period 3 June 2008 to 27 June 2008.  

4.  The applicant provides:

     a.  A Bronze Star Medal Certificate from 1st Theater Sustainment Command (Forward), signed by the Commanding General and annotated as Permanent Order Number 135-225, dated 15 May 2014, which shows he was awarded the Bronze Star Medal for exceptionally meritorious service while serving as an Executive Officer (330th Movement Control Battalion) in support of Operation Enduring Freedom.

     b.  An AAM Certificate from Task Force Gauntlet, Combined Joint Task 
Force-10, annotated with Permanent Orders Number 162-003, dated 11 June 2014, which shows he was awarded an AAM for exceptionally meritorious service in support of Operation Enduring Freedom, from 21 March 2014 to 1 July 2014.  

     c.  An AAM Certificate from the 257th Transportation Battalion (MC), annotated with Permanent Orders Number 09-241-013, dated 30 August 2009, which shows he was awarded an AAM for exceptionally meritorious achievement in support of Operation Shadow Maker, from 17 August 2009 to 28 August 2009.  

     d.  An AAM Certificate from Headquarters, 821st Transportation Battalion (MT), annotated with Permanent Orders 177-04, dated 25 June 2008, which shows he was awarded an AAM for meritorious achievement while assigned to the 993rd Transportation Company, from 3 June 2008 through 27 June 2008.   
     
	e.  A NATO Medal Certificate, signed by The Secretary General of NATO, which awarded him the Non-Article 5 NATO Medal for service with NATO in relation to ISAF operations during the period 16 October 2013 to 16 June 2014.  

5.  Army Regulation 635-8 (Separation Processing and Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for preparation of the DD Form 214.  It states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty.  It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge.  It is important that information entered on the form is complete and accurate.  Chapter 5 contains specific guidance for preparation of the DD Form 214.  It states for block 13, list all Federally-recognized awards and decorations for all periods of service.  Do not use abbreviations. Do not enter foreign or State level awards. 

DISCUSSION AND CONCLUSIONS:

1.  The applicant's request for correction of his DD Form 214 to show his missing awards was carefully considered.  

2.  Certificates show permanent orders awarded him the:

* Bronze Star Medal for meritorious service while serving as an Executive Officer (330th Movement Control Battalion) in support of Operation Enduring Freedom
* AAM for exceptionally meritorious service in support of Operation Enduring Freedom, from 21 March 2014 to 1 July 2014 
* AAM for exceptionally meritorious achievement in support of Operation Shadow Maker, from 17 August 2009 to 28 August 2009
* AAM for meritorious achievement while assigned to the 993rd Transportation Company, from 3 June 2008 through 27 June 2008

3.  His DD Form 214 shows no Bronze Star Medal and only a single AAM; therefore, it would be appropriate to correct his DD Form 214 to show he was awarded the Bronze Star Medal and the AAM (3rd Award).   

4.  He was awarded the NATO Medal for service with NATO in relation to ISAF operations during the period 16 October 2013 to 16 June 2014.  Therefore, it would be appropriate to correct his DD Form 214 to show the NATO Medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending block 13 of his DD Form 214 by deleting the "Army Achievement Medal" and adding the Bronze Star Medal, Army Achievement Medal (3rd Award), and NATO Medal.
