Army Regulation 635-8 (Separation Processing and Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for preparation of the DD Form 214. It states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. It is important that information entered on the form is complete and accurate. Chapter 5 contains specific guidance for preparation of the DD Form 214. It states for block 13, list all Federally-recognized awards and decorations for all periods of service. Do not use abbreviations. Do not enter foreign or State level awards. DISCUSSION AND CONCLUSIONS: 1. The applicant's request for correction of his DD Form 214 to show his missing awards was carefully considered. 2. Certificates show permanent orders awarded him the: * Bronze Star Medal for meritorious service while serving as an Executive Officer (330th Movement Control Battalion) in support of Operation Enduring Freedom * AAM for exceptionally meritorious service in support of Operation Enduring Freedom, from 21 March 2014 to 1 July 2014 * AAM for exceptionally meritorious achievement in support of Operation Shadow Maker, from 17 August 2009 to 28 August 2009 * AAM for meritorious achievement while assigned to the 993rd Transportation Company, from 3 June 2008 through 27 June 2008 3. His DD Form 214 shows no Bronze Star Medal and only a single AAM; therefore, it would be appropriate to correct his DD Form 214 to show he was awarded the Bronze Star Medal and the AAM (3rd Award). 4. He was awarded the NATO Medal for service with NATO in relation to ISAF operations during the period 16 October 2013 to 16 June 2014. Therefore, it would be appropriate to correct his DD Form 214 to show the NATO Medal.
