APPLICANT REQUESTS THAT: 

 

Her last Fitness Assessment (FA) score be removed from her 
records.

APPLICANT CONTENDS THAT: 

 

Through no fault of her own, she failed the PFT; however, she 
provides a signed copy of AF Form 108, Physical Fitness Education 
and Intervention Processing, that states, “Based on medical 
advice regarding her medical condition and the effect of 
prosecutions, this fitness assessment will not count as negative. 
Therefore no administrative action or referral EPR will be 
accomplished.” 

 

In support of her request, the applicant provides a copy of AF 
Form 108.

STATEMENT OF FACTS: 

 

The applicant is currently serving in the Regular Air Force in 
the grade of staff sergeant (E-5). 

 

The applicant’s fitness score, dated 29 Jun 11, was 
unsatisfactory. 

 

The remaining relevant facts pertaining to this application are 
contained in the letters prepared by the appropriate offices of 
the Air Force, which is at Exhibit B.

AIR FORCE EVALUATION: 

 

HQ USAF/A1PP cannot make a recommendation based on the evidence 
provided by the applicant. In this respect, the applicant was 
contacted and afforded the opportunity to provide additional 
supporting documentation; however, the applicant did not respond. 
Without her specific medical conditions and the medications 
prescribed, A1PP cannot verify these had an effect on her 29 Jun 
11 FA.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

The applicant’s Family Practice Physician Assistant submits a 
memorandum on her behalf. She has had a 1.5 year history of 
lumbago likely caused by lumbar radiculopathy, herniated disc at 
L3-L4, sacrolitis, and spondylitis. Additionally, she has a 
history of rheumatoid arthritis in her hips. She has been seen 
by specialists including orthopedics, neurology, rheumatology, 
and pain management. The Physician Assistant confirms she 
suffered from these medical conditions and was being treated at 
the time of her FA test on 29 Jun 11.

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by existing 
law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice. After a review 
of the evidence of record and the applicant’s submission we 
believe that relief is warranted. We note the applicant’s Family 
Practice Physician Assistant confirms the applicant suffered from 
a number of debilitating medical conditions during the time of 
her fitness assessment test on 29 June 2011. The Physician 
Assistant also advises the applicant is undergoing a medical 
evaluation board. As such we believe it would be in the interest 
of justice to remove the unsatisfactory fitness assessment score 
from her record. Therefore, we recommend that the records be 
corrected as indicated below.

THE BOARD RECOMMENDS THAT: 


 

The pertinent military records of the Department of the Air Force 
relating to APPLICANT be corrected to show that her Fitness 
Assessment that was administered on 29 June 2011 be declared 
void.
