APPLICANT REQUESTS THAT: 

 

1. His fitness assessment (FA) score recorded on 9 August 2011 
be removed from the Air Force Fitness Management System (AFFMS). 

 

2. The three previous scores be considered for removal from 
AFFMS due to current medical findings.

APPLICANT CONTENDS THAT: 

 

In the cardio portion of his FA he unexpectedly experienced back 
pain during the run. 

 

He is currently diagnosed with Lumbago and severe back pain. His 
back pain started in early 2010. 

 

In support of his request, the applicant provides a copy of his 
AF Form 108, Physical Fitness Education and Intervention 
Processing, physical therapy letter, and supporting medical 
documents.

STATEMENT OF FACTS: 

 

The applicant is currently serving in the Regular Air Force in 
the grade of Senior Airman (SrA), E-4. 

 

The remaining relevant facts pertaining to this application are 
contained in the letter prepared by the appropriate office of 
the Air Force at Exhibit B. Accordingly there is no need to 
recite these facts in this Record of Proceedings.

AIR FORCE EVALUATION: 


 

HQ USAF/A1PP recommends partial relief. A1PP recommends 
removing the 9 August 2011 FA score from AFFMS due to supporting 
documentation from the applicant’s evaluating physician and 
treating physical therapist. However, they do not recommend 
removing any FA scores prior to August 2011 since the 
applicant’s back pain was not documented until August 2011.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the 
applicant on 29 December 2011 for review and comment within 
30 days. As of this date, no response has been received by this 
office.

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by 
existing law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice. After a review 
of the evidence of record and the applicant’s submission we 
believe that full relief is warranted. The Board notes the Air 
Force office of primary responsibility recommends partial relief 
by only removing the 9 August 2011 “Unsatisfactory” Fitness 
Assessment score and do not recommend removing any scores prior 
to August 2011 since the applicant’s medical condition was not 
documented until August 2011. However, we note the Military 
Medical Provider concludes there are medical conditions which 
preclude or have precluded the applicant from achieving a 
passing fitness assessment score in the last two years. Based 
on the medical provider’s assessment, we believe the three 
scores rendered prior to the August 2011 test should also be 
removed. Therefore, we recommend that the applicant’s record be 
corrected as indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air 
Force relating to APPLICANT be corrected that the Fitness 
Assessment (FA) scores dated 1 December 2010, 24 February 2011, 
10 May 2011 and 9 August 2011 be removed from the Air Force 
Fitness Management System.
