APPLICANT REQUESTS THAT:
His Joint Service Achievement Medal (JSAM) be placed into his master
personnel records and updated into the military personnel data system.

APPLICANT CONTENDS THAT:
The Joint Theater Missile Defense-Attack Operations (JTMD-AO) submitted the
decoration and mailed it directly to his unit. For reasons unknown, the
decoration was not immediately updated into his record and the publishing
orders were apparently misplaced and no notification was provided.
In support of his request, applicant provided a copy of the JSAM and
supporting statements. His complete submission, with attachments, is at
Exhibit A.

STATEMENT OF FACTS:
Data extracted from the personnel data system reflects applicant was
appointed a second lieutenant on 11 Aug 95. He has been progressively
promoted to the grade of major, having assumed that grade effective and
with a date of rank of 1 Jan 06. He is currently serving on active duty as
Chief, European Engagement at Ramstein AB GE.

AIR FORCE EVALUATION:
AFPC/DPPPR recommends denial. DPPPR states the special order is the
official document that authorizes the decoration. Without the special
order, entitlement cannot be verified.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant on 27 Jan
06 for review and comment within 30 days. As of this date, this office has
received no response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was not timely filed; however, it is in the interest of
justice to excuse the failure to timely file.
3. Sufficient relevant evidence has been presented to demonstrate the
existence of an error or injustice. In this respect, the applicant
requests the JSAM that he received be updated into the personnel data
system and placed into his military records. He contends his unit received
the JSAM without decoration orders. In support of his request, applicant
provided a statement from the award approval authority indicating he
approved and signed the award on 16 Nov 98. It appears that the applicant
has made considerable effort to locate the order awarding the JSAM, but has
been unable to do so. In view of the above, and having no reason to doubt
the integrity of the former senior Air Force officer involved, we believe
that to deny the applicant credit for the recognition he received for his
outstanding contributions is an injustice and further believe correction of
his record to include the JSAM is warranted. Accordingly, we recommend his
records be corrected as indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force relating
to APPLICANT be corrected to show that on 16 November 1998, he was awarded
the Joint Service Achievement Medal for meritorious achievement during the
period 1 May 1998 through 30 September 1998.

MEMORANDUM FOR THE CHIEF OF STAFF
Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of Section
1552, Title 10, United States Code (70A Stat 116), it is directed that:
The pertinent military records of the Department of the Air Force
relating to APPLICANT, be corrected to show that on 16 November 1998, he
was awarded the Joint Service Achievement Medal for meritorious achievement
during the period 1 May 1998 through 30 September 1998.

The DPPPR evaluation is at Exhibit B. ARPC/DPPPO recommends the Board deny the applicant’s request for SSB consideration. It is the officer’s responsibility to ensure the accuracy of his record prior to the board convening date, not after he has been nonselected for promotion. After reviewing the evidence of record, we note the applicant’s OSR did not contain a copy of his DMSM citation, nor did his OSB reflect his award of his AAM/8OLC at the time the board convened.

Therefore he will be awarded the KDSM and his records changed to reflect such; however, DPPPR recommends award of the JUMA be denied. We took notice of the applicant's complete submission in judging the merits of the case; however, we agree with the opinion and recommendation of the Air Force office of primary responsibility and adopt its rationale as the basis for our conclusion that the applicant has not been the victim of an error or injustice.
