APPLICANT REQUESTS THAT:
Her records be corrected to show she was awarded the Joint Service
Achievement Medal (JSAM) for the period 3 December 1996 to 17 April
1997.

APPLICANT CONTENDS THAT:
She was awarded the JSAM during Operation Pacific Haven, Anderson Air
Force Base, Guam, from 3 December 1996 to 17 April 1997; however, the
Special Order was not provided with the certificate and citation. The
JSAM was listed in her records and was pulled upon determining she
didn’t have orders for it.
In support of her request, the applicant submits a copy of a JSAM
Certificate, a Citation for the JSAM and a photograph of her receiving
an award.

STATEMENT OF FACTS:
The applicant’s records reflect she was commissioned a second
lieutenant in the Regular Air Force on 29 June 1992. She currently
serves in the grade of major.
Her records reflect award of the Meritorious Service Medal, the Air
Force Commendation Medal, w/3 Oak Leaf Clusters (OLCs), the Air Force
Achievement, w/1 OLC, and the Joint Service Achievement Medal, w/1
OLC.

AIR FORCE EVALUATION:
AFPC/DPPPR recommends denial. DPPPR states the Special Order is the
official document that authorizes decorations. Without the approved
Special Order, her entitlement to the JSAM cannot be verified.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant on 9
June 2006 for review and comment within 30 days. As of this date, no
response has been received by this office.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Sufficient relevant evidence has been presented to demonstrate
the existence of error or injustice. After a thorough review of the
applicant’s submission and the available evidence of record, the board
is persuaded she should be awarded the JSAM. The Board notes, the
applicant provided a photograph of her being presented with the JSAM
as well as a copy of a citation covering the period in question. In
view of the above, and to offset the possibility of an injustice, we
believe that any doubt in this case should be resolved in favor of the
applicant. Accordingly, we recommend that her records be corrected to
the extent indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT, be corrected to show that:
a. On 22 April 1997, she was awarded the Joint Service Achievement
Medal, First Oak Leaf Cluster.
b. On 4 January 2006, she was awarded the Joint Service
Achievement Medal, Second Oak Leaf Cluster, rather than the Joint
Service Achievement Medal, Basic.

MEMORANDUM FOR THE CHIEF OF STAFF
Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of
Section 1552, Title 10, United States Code (70A Stat 116), it is directed
that:
The pertinent military records of the Department of the Air Force
relating to XXXXXXXXXXXXXX, be corrected to show that:
a. On 22 April 1997, she was awarded the Joint Service
Achievement Medal, First Oak Leaf Cluster, for the period 3 December 1996
to 17 April 1997.
b. On 4 January 2006, she was awarded the Joint Service
Achievement Medal, Second Oak Leaf Cluster, for the period 14 July 2005
to 1 December 2005, rather than the Joint Service Achievement Medal,
Basic.
