APPLICANT REQUESTS THAT:
The social security number (SSN) on his DD Form 214 be changed from
***-**-4662 to ***-**-6631.

APPLICANT CONTENDS THAT:
According to the Social Security Administration, his correct social
security number is ***-**-6631.
In support of his request, the applicant submits a copy of his DD Form
214 and verification documents from the Social Security Office. The
applicant’s complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:
Applicant's military personnel records reflect he enlisted in the
Regular Air Force on 22 June 1973 for a period of six years. He was
honorably discharged on 14 May 1974 under the provisions of AFM 39-10
(convenience of the government). He had completed a total of 10
months and 23 days and was serving in the grade of airman first class
(E-3) at the time of separation.
Documents contained in the applicant’s military personnel records
(MPR) reflect his SSN as ***-**-4662.

AIR FORCE EVALUATION:
HQ AFPC/DPSFCMP recommends the application be denied. DPSFCMP states
that the applicant’s SSN on his enlistment contract was ***-**-4662
and that he served under this SSN. The applicant was asked to provide
a letter from the Social Security Administration stating the requested
SSN was indeed his SSN on the date of enlistment. The applicant did
not respond to their request. The AFPC/DPSFCMP evaluation is at
Exhibit C.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to applicant on 7 May
2004 for review and response. As of this date, no response has been
received by this office (Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law
or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to demonstrate
the existence of error or injustice. After a thorough review of the
evidence of record and applicant’s submission, we are unpersuaded that
the requested change should be granted. We note that the DD Form 214,
issued on 14 May 1974, accurately reflects the applicant’s SSN while
serving on active duty. No evidence has been presented from the
Social Security Administration to indicate the applicant’s SSN at the
time of his enlistment was incorrect. In view of the foregoing, we
find no compelling basis to recommend granting the relief sought in
this application.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that the
application was denied without a personal appearance; and that the
application will only be reconsidered upon the submission of newly
discovered relevant evidence not considered with this application.
