APPLICANT REQUESTS THAT:
The social security number (SSN) on his DD Form 214 be changed from
***-**-4601 to ***-**-4506.

APPLICANT CONTENDS THAT:
His social security number was incorrectly entered on his DD Form 214.
He requested a change upon discharge, but it apparently was not
changed.
In support of his request, the applicant submits a copy of his social
security card. The applicant’s complete submission, with attachment,
is at Exhibit A.

STATEMENT OF FACTS:
Applicant's military personnel records reflect he enlisted in the
Regular Air Force on 3 September 1982 for a period of four years. He
was released from active duty on 13 May 1986 under the provisions of
AFR 39-10 (early separation program - strength reduction) and
transferred to the Air Force Reserve. He had completed a total of 3
years, 6 months and 11 days and was serving in the grade of senior
airman (E-4) at the time of separation.
On 16 August 1986, applicant enlisted in the Air National Guard (ANG)
and Reserve of the Air Force in the grade of E-4 for a period of six
years. He continued to serve until he was relieved from his
assignment with the Pennsylvania ANG and honorably discharged,
effective 5 April 1993. On 6 April 1993, he was transferred to HQ
ARPC/IRRS. The ANG/USAFR Point Credit Summary, prepared 17 July 1993,
reveals the applicant completed a total of 10 years, 8 months and 11
days of satisfactory Federal service. On 14 September 1994, he was
relieved from his Reserve assignment (Nonobligated Nonparticipating
Ready Personnel Section) and honorably discharged from the Air Force
Reserve in the grade of staff sergeant (E-5).
During the time frame of 1982 - 1990, the documents contained in the
applicant’s military personnel records (MPR) reflect his SSN as ***-**-
4601. Subsequent documents reveal applicant’s SSN as ***-**-4506.

AIR FORCE EVALUATION:
HQ AFPC/DPSFCMP recommends the application be denied. DPSFCMP states
that the applicant’s SSN on his enlistment contract was ***-**-4601
and that he served under this SSN. The applicant was asked to provide
a letter from the Social Security Administration stating the requested
SSN was indeed his SSN on the date of enlistment. The applicant did
not respond to their request. The AFPC/DPSFCMP evaluation is at
Exhibit C.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to applicant on 7 May
2004 for review and response. As of this date, no response has been
received by this office (Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law
or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to demonstrate
the existence of error or injustice. After a thorough review of the
evidence of record and applicant’s submission, we are unpersuaded that
the requested change should be granted. We note that the DD Form 214,
issued on 13 May 1986, accurately reflects the applicant’s SSN while
serving on active duty. No evidence has been presented from the
Social Security Administration to indicate the applicant’s SSN at the
time of his enlistment was incorrect. In view of the foregoing, we
find no compelling basis to recommend granting the relief sought in
this application.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that the
application was denied without a personal appearance; and that the
application will only be reconsidered upon the submission of newly
discovered relevant evidence not considered with this application.

On 25 April 1982, he was honorably discharged from the Army Reserve for completion of required service. On 1 December 1986, the discharge authority approved the recommended separation and directed that the applicant be discharged with a general (under honorable conditions) discharge. He has changed over the years and he wants the chance to correct his mistakes.

The applicant entered the Technical School at Keesler AFB, MS on 8 January 1986, to complete the Air Traffic Control Operator course E3ABR27230. This extension would normally satisfy the service retainability for retraining requirements only if the controlled duty assignment (CDA) for the ATC course was less than 23 months and technical school completion date was prior to his initial date of separation of 6 May 1986.

He was presented to a Medical Evaluation Board (MEB) due to chronic pain in his left tibia. The AFPC/DPPRS evaluation, with attachments, is at Exhibit D. AFPC/DPPAE states the RE Code assigned at the time of his 6 December 1994 medical discharge is correct.
