THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to add three Army Commendation Medals and the Afghanistan Campaign Medal.
2. The applicant states:
* she was awarded three Army Commendation Medals
* her service in Afghanistan from July 2013 through July 2014, as listed on her DD Form 214, meets the requirements for award of the Afghanistan Campaign Medal
3. The applicant provides:
* three DA Forms 4980-14 (Army Commendation Medal Certificate)
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 13 June 2011.
2. She provided three DA Forms 4980-14, containing the following information:
* Permanent Order Number 002-001 awarded her the Army Commendation Medal on 2 January 2014 for exceptional and meritorious service during Operation Enduring Freedom from 10 July 2013 through 9 July 2014
* Permanent Order Number 098-005 awarded her the Army Commendation Medal on 8 April 2014 for exceptional and meritorious achievement as the Noncommissioned Officer in Charge during the 2014 Afghan Presidential Elections from 3 April 2014 through 5 April 2014
* Permanent Order Number 349-003 awarded her the Army Commendation Medal on 15 December 2014 for exceptionally meritorious service while assigned as an intelligence analyst from 6 January 2012 through 22 January 2015
3. On 22 January 2015, she was honorably released from active duty. Her DD Form 214 shows she served in Afghanistan in a designated imminent danger pay area from 10 July 2013 through 4 July 2014. She participated in the Transition I (1 July 2011-31 December 2014) campaign during this period. Her DD Form 214 does not show she was awarded or authorized the Army Commendation Medal or the Afghanistan Campaign Medal.
4. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. It states the Afghanistan Campaign Medal is awarded to members who have served in direct support of Operation Enduring Freedom. The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of Operation Enduring Freedom. A bronze service star is authorized for wear with this medal for participation in each credited campaign.
DISCUSSION AND CONCLUSIONS:
1. She was awarded the Army Commendation Medal for the periods 10 July 2013 through 9 July 2014, 3 through 5 April 2014, and 6 January 2012 through 22 January 2015. She provided documentation showing approval of the awards, including the issuance of permanent orders.
2. She deployed to Afghanistan in support of Operation Enduring Freedom from 10 July 2013 through 4 July 2014 and participated in one campaign which is qualifying service for award of the Afghanistan Campaign Medal with one bronze service star.

BOARD DETERMINATION/RECOMMENDATION:
