THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her additional skill identifier (ASI) be added to her DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states that she attended the ASI N4 (Health Physics Specialist) course and worked as a health physics technician for 3 years at Walter Reed Army Medical Center.  

3.  The applicant provides her DD Form 214; her Enlisted Record Brief, and a DA Form 1059 (Service School Academic Evaluation Report).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 2 March 2004.  She completed basic training and advanced individual training and was awarded military occupational specialty (MOS) 91S (Preventive Medicine Specialist), later converted to MOS 68S.

2.  On 5 May 2005, the applicant completed the 12-week Health Physics Specialty course.  Her Enlisted Record Brief, dated 16 October 2007, shows she had been awarded ASI N4.

3.  The applicant was honorably released from active duty on 1 March 2008.  Item 11 (Primary Specialty) of her DD Form 214 shows her primary MOS of 68S2O but does not show her ASI of N4.  Item 14 (Military Education) does not show she completed the Health Physics Specialty course.  Her Enlisted Record Brief does not show she completed this course.

4.  Army Regulation 635-5 prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  In pertinent part, it states that for enlisted Soldiers the first five characters of the primary MOS code, which includes the three characters of the MOS, the fourth character of skill and grade level in the MOS, and the fifth character of a special qualification identifier (SQI), will be entered in item 11.

5.  Army Regulation 635-5 also states that, from the Enlisted Record Brief/Officer Record Brief, formal in-service training courses successfully completed during the period of service covered by the DD Form 214 will be entered in item 14.

DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded ASI N4 after completing the Health Physics Specialty course.

2.  The regulatory guidance for completing item 11 of the DD Form 214 does not specifically require that an ASI be entered in item 11; however, as an exception to policy it would be appropriate to add it to item 11 of the applicantÂ’s DD Form 214.

3.  It is also noted that the Health Physics Specialty course is not listed in item 14 of the applicantÂ’s DD Form 214.  Although it may not have been added to her DD Form 214 because it was not listed on her Enlisted Record Brief, the course should have been listed on her Enlisted Record Brief.  Therefore, it would also be appropriate to add this course to item 14 of her DD Form 214 at this time.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

     a.   amending item 11 of her DD Form 214 to add ASI N4; and

     b.  amending item 14 of her DD Form 214 to add the 12-week Health Physics Specialty course, completed in 2005.
