THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty):
* amend the primary military occupational specialty (PMOS) shown in item 11 (Primary Specialty) from 68W3O (Health Care Specialist) to 68W3OM6 (Health Care Specialist with additional skill identifier (ASI) for practical/vocational nurse)
* change the years and months shown for his PMOS
* remove the entry in item 14 (Military Education) showing attendance at Practical Nurse Course
2. The applicant states:
* he served in the Army in the MOS 68W3OM6
* he enlisted on 18 July 1996 in MOS 91C (Licensed Practical Nurse) under the Stripes for Skills program, which was offered at that time
* in 2002, the Army consolidated MOS' 91C and 91B (Medical Specialist); his MOS then became 68W3OM6
* he was a civilian nurse when he enlisted; his DD Form 214 should therefore show that he held his PMOS for his entire term of active service (15 years, 9 months, and 19 days)
3. The applicant provides:
* Student Final Record, McFatter Vocational-Technical Center showing successful completion of the Practical Nursing Program
* Orders Number 132-0114, dated 11 May 2012, issued by Headquarters, U.S. Army North (Fifth Army) showing assignment to the Transition Point for final outprocessing
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 30 July 1996. His records are not complete in that his initial enlistment documents are missing. His Enlisted Record Brief (ERB), however, shows he attended basic combat training (BCT) at Fort Sill, OK, and was then assigned directly to the U.S. Army Medical Department Activity (MEDDAC), Fort Benning, GA as a Practical Nurse. He is not shown as having attended advanced individual training prior to that assignment.
2. His ERB also shows his PMOS as 68W with the ASI of M6. Attendance at a Practical Nurse Course is not reflected.
3. He was honorably discharged on 18 May 2012. His DD Form 214 shows he completed 15 years, 9 months, and 19 days of net active creditable service.
a. Item 11 shows 68W3O, Health Care Specialist - 14 years, 5 months.
b. Item 14 lists Practical Nurse Course, 52 weeks, 1997.
4. His official military personnel file (OMPF) is void of any documents showing either attendance or completion of the Practical Nurse Course. It does, however, indicate he was assigned to the U.S. Army MEDDAC at Fort Benning during the timeframe his DD Form 214 states he attended the course. It contains the following documents:
a. Orders Number 105-00208, dated 15 April 1997, issued by Headquarters, U.S. Army Infantry Center, which show the applicant was promoted from specialist (SPC) to sergeant (SGT) effective 12 December 1996. It additionally shows he was awarded his PMOS effective the same date as his promotion.
b. His record contains two DA Forms 2166-7 (Noncommissioned Officer (NCO) Evaluation Report (NCOER)) which show the applicant was evaluated as a SGT in the duty position of Practical Nurse for the rated periods of December 1996 through July 1997 and August 1997 through March 1998.
c. Neither report makes any mention of the applicant's attendance at a Practical Nurse Course.
5. Army Regulation 611-1 (Military Occupational Classification Structure Development and Implementation) addresses skill levels associated with MOS. It states:
a. The first three characters are a numeric-alpha combination that identify the MOS.
b. The fourth character is a number and shows the skill and grade level for the MOS.
c. The fifth character may be a letter or number and shows a skill qualification identifier (SQI).
d. The sixth and seventh characters may be alphanumeric and represent ASI.
6. Deputy Chief of Staff, G-1 Smartbook lists skill levels and ASI for all MOS in the Army. It shows, in pertinent part, ASI M6 identifies a Practical/Vocational Nurse for skill levels 1 to 4 only.
7. Army Regulation 635-8 (Separation Processing and Documents) prescribed policies and procedures for the use and completion of the DD Form 214.
a. For PMOS, enter the titles of all MOSs awarded and held for at least 1 year during the current period of service, and include for each MOS the number of years and months held. A PMOS is awarded by Service school completion or the publication of orders awarding the MOS. For enlisted Soldiers, specify the first five characters of the MOS. A single ASI may be listed immediately following the five-character MOS.
b. The item addressing military education will show all formal in-service (full-time attendance) training courses successfully completed during the period covered by the DD Form 214 with a duration of at least 1 week or 40 hours. Include title, length in weeks, and year completed. Acceptable source documents include the ERB, DA Form 1059 (Service School Academic Evaluation Report), or other military-issued certificate of complete with from and through dates or the number of weeks.
DISCUSSION AND CONCLUSIONS:
1. The applicant requests his DD Form 214 be amended to correctly show his PMOS, correct the number of years and months he held his PMOS, and remove the reference to the completion of the Practical Nurse Course.
2. His OMPF is void of documents showing his initial enlistment and the award of his PMOS. His ERB, however, lists his PMOS as 68W, with an ASI of M6. It would therefore be appropriate to amend his DD Form 214 to show his PMOS as 68W3OM6 and it should be so corrected.
3. The applicant's OMPF shows orders which both promoted the applicant and awarded him his PMOS effective 12 December 1996. Based on these orders, he held his PMOS for a period of 15 years and 5 months. His DD Form 214 should therefore be corrected accordingly.
4. Although reflected on his DD Form 214, his record contains no documentation affirming his attendance at the Practical Nurse Course. Additionally, two NCOERs from the period during which his DD Form 214 states he attended the course fail to mention his attendance. It is reasonable to assume a course shown as 52 weeks in length would merit being mentioned in at least one of the two NCOERs. Thus, it is highly likely the entry made on his DD Form 214 was an error and should be removed.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by correcting his DD Form 214 for the period ending 18 May 2012 by amending it as follows:
a. Item 11 as follows:
* delete "68W3O HEALTH CARE SPECIALIST - 14 YRS 5 MOS//NOTHING FOLLOWS"
* add "68W3OM6 HEALTH CARE SPECIALIST, PRACTICAL/VOCATIONAL NURSE - 15 YRS, 5 MOS//NOTHING FOLLOWS"
b. Item 14, delete the entry showing "PRACTICAL NURSE COURSE 52 WEEKS, 1997."

APPLICANT REQUESTS: The applicant requests that her records be corrected to show that she was promoted to sergeant effective and with a date of rank of 27 June 1997. There is no evidence of any proficiency training completed, nor any evidence that she was recommended for promotion by her prior unit commanders. Consequently, and notwithstanding the recommendation made by her current hospital commander, there is insufficient evidence to grant the applicant's request.

On 10 August 1996, the applicant requested transfer to the USAR and on 9 October 1996 he enlisted in the USAR in pay grade E-4. In a 21 December 2001 memorandum to the Total Army Personnel Command (PERSCOM) the MEDDAC commander at Fort Stewart recommended that the applicant be promoted to sergeant effective on 26 July 2001 as an exception to policy. Both the applicant’s company commander and MEDDAC commander recommended that the applicant be promoted to sergeant effective on 26 July 2001, the date that she completed the required proficiency training.
