         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that her Certificate of Release or Discharge from Active Duty, DD Form 214, be amended to add the second award of the Army Good Conduct Medal, the Army Achievement Medal (AAM), the Army Commendation Medal (ARCOM), the National Defense Service Medal (NDSM), and the Noncommissioned Officers Professional Development Ribbon (NCOPDR) in item 13 and the 1998 Mental Health Specialist course, the 4 week 1999 Primary Leadership Development Course (PLDC), and Additional Skill Identifier (ASI) M8 Drug and Alcohol Counseling courses be added to item 14.

3. The applicant states, in effect, that she was awarded or is entitled to award of the second award of the Army Good Conduct Medal, the AAM, the ARCOM, the NDSM, and the NCOPDR and she completed the Mental Health Specialist, PLDC, and ASI M8 Drug and Alcohol Counseling courses.

4. The applicant’s military records are not available. Information contained herein was obtained from alternate sources.

5. The applicant enlisted in the Regular Army on 4 December 1992. A completion certificate shows she completed the 2-week U. S. Army Drug and Alcohol Rehabilitation Training (USADART) (Group) Course on 6 March 1998. A memorandum dated 5 November 1998 indicates she completed the Mental Health train-up program. Orders 138-003, Madigan Army Medical Center dated 18 May 1999 awarded her the AAM. A Service School Academic Evaluation Report, DA Form 1059, shows she completed the 4-week PLDC on 4 June 1999. Permanent Orders 202-1, Madigan Army Medical Center dated 21 July 1999 shows she was awarded the Army Good Conduct Medal 2d award for the period 4 December 1995 to 3 December 1998. A completion certificate shows she completed the 2-week U. S. Army Drug and Alcohol Rehabilitation Training (Individual) Course on 6 October 2000. On or about 7 November 2000, she requested award of ASI M8 (Drug and Alcohol Counseling). Orders 334-013, Madigan Army Medical Center dated 29 November 2000 awarded her the ARCOM.

6. The applicant was discharged on 19 February 2001. Her DD Form 214, item 11 shows her primary military occupational specialty as 91X10M8. Item 13 shows her awards and decorations as the Army Service Ribbon, the Army Good Conduct Medal (basic), the Overseas Service Ribbon, and the Expert Marksmanship Qualification Badge with Rifle Bar. Item 14 shows her military education as the 1993 3-week General Medical Orientation course, the 1993 12-week Psychiatric Specialist Course, the 1998 2-week USADART Group Counseling course, and the 2000 2-week Drug and Alcohol rehabilitation Training course.

7. Army Regulation 600-8-22 prescribes Army policy and criteria concerning individual military awards. It provides for award of the NDSM for honorable active service for any period between 2 August 1990 and 30 August 1995. It provides for award of the NCOPDR upon successful completion of designated NCO professional development courses. Successful completion of PLDC qualifies for award of the basic ribbon.

8. Army Regulation 635-5 prescribes the separation documents prepared for soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. In pertinent part it states that formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 will be entered in item 14.

9. The U. S. Total Army Personnel Command, Health Services Branch stated that the 91X train-up program was not a formal course but was an exportable course provided to soldiers transitioning into MOS 91X.

CONCLUSIONS
:

1. The applicant provided evidence to show she was awarded the second award of the Army Good Conduct Medal, the AAM, and the ARCOM and to show she met the eligibility criteria for award of the NDSM and the NCOPDR. These awards and decorations should be added to her DD Form 214.

2. The applicant provided evidence to show she attended PLDC. This course should be added to her DD Form 214.

3. It appears that the Mental Health train-up program the applicant completed was not a formal in-service course and so is not authorized for entry on the DD Form 214.

4. ASI M8 is already listed on the applicant’s DD Form 214 in item 11. The Drug and Alcohol Counseling courses that led to award of that ASI are already listed in item 14.

5. In view of the foregoing, the applicant’s records should be corrected as recommended below.

RECOMMENDATION:

1. That the applicant’s DD Form 214 be amended:

a. to add the second award of the Army Good Conduct Medal, the Army Achievement Medal, the Army Commendation Medal, the National Defense Service Medal, and the Noncommissioned Officers Professional Development Ribbon to item 13; and
b. to add the Primary Leadership Development Course, 4 weeks, 1999 to item 14.

2. That so much of the application as is in excess of the foregoing be denied.
