      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that one of two sets of orders awarding him his
second Army Good Conduct Medal (Permanent Orders Number 174-19), and orders
for the Army Achievement Medal (Permanent Orders Number 310-04) be removed
from his Official Military Personnel File (OMPF).

2.  The applicant states that orders for award of his second Army Good
Conduct Medal (AGCM) were published twice; once by his losing command and
once by his gaining command while he was in-transit to his new duty
station.  He also states that pertinent regulations require the Army
Achievement Medal (AAM) Certificate, with Permanent Orders number be posted
to his OMPF, not the separate Permanent Orders.

3.  The applicant provides a copy of AGCM Permanent Orders Number 204-
00064, Headquarters, United States Army Infantry Center, Fort Benning,
Georgia, dated 27 October 1994; AGCM Permanent Orders Number 174-19, 3rd
Personnel Group, 546th Personnel Service Battalion, Fort Hood, Texas, dated
21 December 1994, and AAM Permanent Orders Number 310-04, Headquarters,
293rd Engineer Combat Battalion (Heavy), APO New York 09034, dated
6 November 1990.

CONSIDERATION OF EVIDENCE:

1.  The applicant's OMPF contains Permanent Orders Number 204-00064,dated
27 October 1994 and Permanent Orders Number 174-19, dated 21 December 1994.
 Both orders awarded him the AGCM (2nd award) for the same period of
service, 2 November 1991 through 1 November 1994.

2.  The applicant's OMPF contains Permanent Orders Number 310-01, dated
6 November 1990, awarding him the AAM for the period 13 November
1988 through 13 November 1990.  His OMPF also contains an AAM Certificate
awarding him the AAM with the same Permanent Orders Number and periods of
service, dated 20 November 1990.

3.  Army Regulation 600-8-104 (Military Personnel Information Management/
Records) provides, in pertinent part, that award certificates will be filed
in the OMPF instead of orders, which are obsolete or no longer used, and
that the orders number will be annotated on the certificate with the
approval authority signature block.


DISCUSSION AND CONCLUSIONS:

1.  The applicant has two sets of orders in his OMPF awarding him the AGCM
for the period of 2 November 1991 through 1 November 1994.  Only one set of
orders is authorized to accomplish this and the applicant has requested
that Permanent Orders Number 174-19, dated 21 December 1994, the later of
the two, be revoked and removed from his OMPF.  Therefore, the requested
orders should be revoked and removed from his OMPF.

2.  The applicant has two sets of orders with the same order number in his
OMPF awarding him the AAM for the period of 13 November 1988 through
13 November 1990.  Only the AAM Certificate with the Permanent Orders
Number annotated on the certificate is required for filing in the OMPF.
Therefore, it would be appropriate at this time to remove Permanent Orders
Number 310-04, dated 13 November 1990, from his OMPF.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
removing and revoking Permanent Orders Number 174-19, dated 21 December
1994, and Permanent Orders Number 310-04, dated 13 November 1990 from the
applicant's OMPF.
