APPLICANT REQUESTS: Correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show two awards of the Army Achievement Medal.
APPLICANT STATES: That she received two awards of the Army Achievement Medal and they are not listed on her DD Form 214.
EVIDENCE OF RECORD: The applicant's military records show:
The applicant enlisted on 4 October 1983 for a period of 3 years. She served as an administrative specialist in Germany from 22 March 1984 through 12 August 1985 and was honorably released from active duty on 3 October 1986. The applicant was separated from the service on temporary records and a Soldier’s Affidavit.
The applicant’s DD Form 214, which was authenticated in her own hand, does not show the Army Achievement Medal as an authorized award.
There are no orders for award of the Army Achievement Medal in the available records.
Item 9 (Awards, Decorations & Campaigns) on the applicant’s DD Form 214 does not show entitlement to any award of the Army Achievement Medal.
Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that the Army Achievement Medal is awarded to any member of the armed forces of the United States, who while serving in a noncombat area on or after 1 August 1981, distinguished themselves by meritorious service or achievement. As with all personal decorations, formal recommendations, approval through the chain of command, and announcement in orders are required.
DISCUSSION: Considering all the evidence, allegations, and information presented by the applicant, together with the evidence of record, applicable law and regulations, it is concluded:
1. The Board considered the applicant’s contention that she received two awards of the Army Achievement Medal. However, there is no evidence of record to support this contention.
2. There is no evidence of record available to the Board, and the applicant has provided no evidence, which shows that she was recommended for or awarded the Army Achievement Medal. Therefore, the Board determined that there is insufficient evidence on which to base any award of the Army Achievement Medal in this case.
3. In order to justify correction of a military record the applicant must show to the satisfaction of the Board, or it must otherwise satisfactorily appear, that the record is in error or unjust. The applicant has failed to submit evidence that would satisfy this requirement.
4. In view of the foregoing, there is no basis for granting the applicant's request.
DETERMINATION: The applicant has failed to submit sufficient relevant evidence to demonstrate the existence of probable error or injustice.
