         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

         Exhibit A - Application for correction of military
records
         Exhibit B - Military Personnel Records (including
         advisory opinion, if any)

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) dated 27 November 2001, to show award of the Joint Service Commendation Medal.

3. The applicant submits a copy of her Joint Service Commendation Medal certificate, a copy of her Permanent Orders 283-012 issued by Headquarters, U.S. European Command authorizing award of the Joint Service Commendation Medal.

4. The applicant's military records are limited to the documents she has submitted. The available records show she enlisted in the Regular Army for
4 years on 28 November 1995.

5. The Joint Service Commendation Medal was issued to the applicant on
10 October 2001, for meritorious service from 1 June 1998 to 30 September 2001.

6. On 27 November 2001 she was honorably separated as an Information Systems Operator/Analyst (74B10) and transferred to the United States Army Reserve Control Group (Reinforcement). She was credited with 6 years of active service and no lost time. The highest grade she attained was pay grade E-4.

7. At the time of her separation, his DD Form 214, item 13 showed the following
awards; Joint Meritorious Unit Award, NATO Medal, the Army Achievement Medal, Good Conduct Medal, (2nd Award), National Defense Service Medal, Armed Forces Expeditionary Medal, Humanitarian Service Medal, Army Service Ribbon, and Overseas Service Ribbon.

CONCLUSIONS:

1. The evidence of record substantiates that the applicant was awarded the Joint Service Commendation Medal. This award should be added to her
DD Form 214.

2. In view of the foregoing, the applicant’s records should be corrected as recommended below.


RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected by adding the Joint Service Commendation Medal award to item 13 on her
DD Form 214 issued to her on 27 November 2001.
