      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that her records be corrected to
show all of her authorized awards.

2.  The applicant states that she should have received the Good Conduct
Medal and awards related to service in Germany, Kuwait and Iraq.  She
states that she was awarded three Army Achievement Medals and two Army
Commendation Medals and that she should have received the Good Conduct
Medal.  She believes that her records are incomplete because she returned
to the United States before her unit did.

3.  The applicant provides copies of her DD Form 214 (Certificate of
Release or Discharge from Active Duty) and the recommendation/approval
forms for two awards of the Army Commendation Medal and three awards of the
Army Achievement Medal.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted and entered active duty on 28 September 1999.
She completed training as a multimedia illustrator and was stationed in
Germany.  She was separated from active duty with an honorable
characterization of service on 30 October 2003 and was transferred to the
United States Army Reserve (USAR) Control Group (Reinforcement) as a
specialist (E4).  The applicant had 4 years, 1 month, and 3 days of
creditable service and 2 years, 8 months, and 10 days of foreign service.

2.  Her DD Form 214 lists her authorized awards as the Army Commendation
Medal (2nd Award), the Army Achievement Medal (3rd Award) and the Army
Service Ribbon.

3.  The applicant’s DA Form 2-1 (Personnel Qualification Record) lists her
awards as “ASR/AAM-1/AAM-2/AAM-3/ARCOM.”  Whether the numerals were meant
to convey the total number of awards or the number of Oak Leaf Clusters is
unclear.

4.  The earlier of the two DA Forms 638 (Recommendation for Award), that
the applicant submitted, awarded her the Army Commendation Medal for the
period 19 March 2003 through 30 May 2003 for meritorious achievement during
Operation Iraqi Freedom.  The form lists her previous awards as the Army
Achievement Medal with three Oak Leaf Clusters.

5.  The later DA Form 638 awarded her the Army Commendation Medal as an end
of tour award for meritorious service from 22 February 2001 to 30 October
2003.  This form also indicates that the applicant’s prior awards are the
Army Achievement Medal and three Oak Leaf Clusters.

6.  The DA Forms 638 for the three Army Achievement Medals include one for
the applicant’s performance on an exercise in Kuwait.  These
recommendations are ambiguous in that they do not consistently identify the
number of previous awards nor do they identify the number of the oak leaf
cluster actually approved. There are, however, a total of three
recommendations for three distinct time periods and no indication that the
applicant was recommended for a fourth award of the Army Achievement Medal
[with three Oak Leaf Clusters].

7.  There is no discreditable information of record.  All of the award
recommendations praise the applicant as being more than simply an
outstanding
Soldier.  One credits her with simultaneously fulfilling the requirements
of two full time staff sergeant positions plus those of a sergeant first
class assignment.

8.  Army Regulation 600-8-22 (Military Awards) provides that the Good
Conduct Medal is awarded to individuals who distinguish themselves by their
conduct, efficiency and fidelity during a qualifying period of active duty
enlisted service.  This period is 3 years except in those cases when the
period for the first award ends with the termination of a period of Federal
military service.  Although there is no automatic entitlement to the Good
Conduct Medal, disqualification must be justified.

9.  Army Regulation 600-8-22 (Military Awards) as amended provides that the
National Defense Service Medal is awarded for honorable active service for
any period between 27 July 1950 through 27 July 1954, 1 January 1961
through 14 August 1974, 2 August 1990 through 30 November 1995 and 11
September 2001 to a date to be determined.  This regulation also provides
that the second and subsequent awards of the National Defense Service Medal
are denoted by a bronze service star affixed to the National Defense
Service Medal.

10.  Military Awards Branch Message [dated 8 April 2005] provided the
following implementing instructions for award of the Iraq Campaign Medal.
Award of the Iraq Campaign Medal is authorized for service in direct
support of Operation Iraqi Freedom on or after 19 March 2003 to a future
date to be determined by the
Secretary of Defense or the cessation of the operation.  The area of
eligibility encompasses all land area of the country of Iraq and the
contiguous sea area out to 12 nautical miles and the air space above these
areas.  To qualify for award of this campaign medal, service members must
have been assigned or attached to or mobilized with units operating in
these areas of eligibility for 30 consecutive days or for 60 non-
consecutive days or meet one of the following criteria:  a) be engaged in
combat during an armed engagement, regardless of the time in the area of
eligibility; b) be medically evacuated from the area of eligibility for
wounds or injuries resulting from participation in an operation or official
duties; or c)  participates as a regularly assigned air crewmember flying
sorties into, out of, within or over the area of eligibility in direct
support of the military operations [each day of operations counts as one
day of eligibility].  This campaign medal may be awarded posthumously.

11.  Human Resources Command Message (Date Time Group 17 March 2004)
disseminated implementing instructions for award of the Global War on
Terrorism Service Medal.  This award is designated for Soldiers who have
participated in or served in support of Global War on Terrorism Operations
outside of the designated area of eligibility determined for award of the
Global War on Terrorism Expeditionary Medal on or after 11 September 2001
to a date to be determined.  All Soldiers on active duty, including Reserve
Component Soldiers mobilized or National Guard Soldiers activated, on or
after 11 September 2001 to a date to be determined having served 30
consecutive days or 60 nonconsecutive days are authorized this award.

12.  Army Regulation 600-8-22 (Military Awards) shows that the Overseas
Service Ribbon was established by the Secretary of the Army on 10 April
1981.  The regulation states, in pertinent part, that effective 1 August
1981, all members of the Active Army, Army National Guard, and Army Reserve
in an active Reserve status are eligible for the award for successful
completion of overseas tours.  The award may be awarded retroactively to
those personnel who were credited with a normal overseas tour completion
before 1 August 1981 provided they had an Active Army status on or after 1
August 1981 and the overseas service is not recognized with another U.S.
service medal.  Numerals are used to denote the second and subsequent
awards of the Overseas Service Ribbon.

DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded first and second awards of the Army
Commendation Medal and a total of three Army Achievement Medals.  These
awards should be shown on her DD Form 214 as the Army Commendation Medal
with Oak Leaf Cluster and the Army Achievement Medal with 2nd Oak Leaf
Cluster.

2.   The applicant clearly distinguished herself by her offense free
behavior and remarkable duty performance.  She should be awarded the Good
Conduct Medal for the period from 28 September 1999 to 27 September 2002
and it should be listed on her DD Form 214.

3.   The applicant earned the National Defense Service Medal, the Iraq
Campaign Medal, the Global War on Terrorism Service Medal, and the Overseas
Service Ribbon.  They should be authorized and shown on her DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by:

      a.  awarding her the Good Conduct Medal and;


      b.  showing that her authorized awards include the Army Commendation
Medal with Oak Leaf Cluster and the Army Achievement Medal with 2nd Oak
Leaf Cluster, the Good Conduct Medal, the National Defense Service Medal,
the Iraq Campaign Medal, the Global War on Terrorism Service Medal, the
Overseas Service Ribbon, and the Army Service Ribbon.
