APPLICANT REQUESTS THAT:
Her records be corrected to reflect she was awarded the Air Force
Commendation Medal (AFCM), Presidential Unit Citation (PUC), Air Force
Outstanding Unit Award (AFOUA), Air Force Good Conduct Medal, first
Oak Leaf Cluster (AFGCM 1 OLC), National Defense Service Medal (NDSM),
Vietnam Service Medal (VSM), Air Force Longevity Service Award, first
Oak Leaf Cluster (AFLSA 1 OLC) and the Outstanding Airman of the Year
Ribbon.

APPLICANT CONTENDS THAT:
Her DD Form 214 does not list all her awards and decorations.
In support of her appeal, applicant submitted a copy of her DD Form
214, Certificate of Release or Discharge from Active Duty, a
certificate of recognition and a list of missing awards.

STATEMENT OF FACTS:
The applicant served on active duty from 27 Feb 74 through 13 Nov 81.
The applicant’s performance reports reflect she was assigned in
Germany from 26 Oct 75 through 23 Nov 79. She served 7 years, 8
months and 13 days of active duty.
The applicant’s DD Form 214, dated 13 Nov 81 reflects she was awarded
the AFOUA, AFGCM (1 OLC), AFLSA, PUC and the NDSM.

AIR FORCE EVALUATION:
HQ AFPC/DPSIDR recommends denying the applicant’s request for the
AFCM, VSM, OAYR and the AFLSA (1 OLC). DPSIDR was unable to verify
that she was recommended or awarded the AFCM. The OAYR is annotated
on the DD Form 214 when a service member is recognized at the Air
Force level. The applicant received the wing level recognition
certificate, which is not annotated on the separation document. The
applicant is not eligible for the VSM because she enlisted in the
military after the inclusive time period for the award (3 Jul 65
through 28 Mar 73). The AFLSA is awarded to service members who
completed eight years of honorable service. The applicant completed 7
years, 8 months and 13 days of honorable service.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant 14
Sep 07, for review and response within 30 days. As of this date, no
response has been received by this office.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to demonstrate
the existence of an error or an injustice. We took notice of the
applicant's complete submission in judging the merits of the case;
however, we agree with the opinion and the recommendation of the Air
Force office of primary responsibility and adopt its rationale as the
basis for our conclusion that the applicant has failed to sustain her
burden of proof that she has been the victim of an error or an
injustice. The applicant requested award of several medals and
ribbons. Her entitlement to the PUC, AFOUA, AFGCM (1 OLC), NDSM and
the AFLSA has been verified and they are reflected on her DD Form 214.
However, no documentation was found in her military records
indicating she is entitled to any of the additional medals requested.
Therefore, we find no compelling basis upon which to recommend the
requested relief.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that the
application was denied without a personal appearance; and that the
application will only be reconsidered upon the submission of newly
discovered relevant evidence not considered with this application.

AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS The AFPC/DPSIDR evaluation is at Exhibit C.
