THE APPLICANT CONTENDS THAT:
He requests his HOR be changed to his permanent address. His current HOR
that is on file is incorrect.

STATEMENT OF FACTS:
Applicant contracted his enlistment in the Regular Air Force on 12 March
2003. DD Form 4/1, Enlistment/Reenlistment Document, reflects
, as his HOR.
EXAMINER'S NOTE: HOR is defined as the place recorded as the home of the
individual at the time they enlisted, were commissioned, or initially
ordered to active duty. It is recorded in the military personnel record to
determine transportation entitlements upon separation. The HOR can be
changed only if there has been a break in service of one full day (inactive
reserve time is not counted as a break in service) or a bona fide error was
made in recording the information given by the member.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate the
existence of an error or injustice. Applicant’s contentions are duly
noted. The applicant is requesting HOR be changed to his permanent address
because the HOR on file is incorrect. The applicant has not provided
evidence which indicates the HOR currently reflected in his records was
incorrectly recorded at the time he initially entered active duty.
However, the Board recommends the requested relief be granted based on the
fact the applicant is requesting a change from one city to another in the
same state, therefore, it would not be any added cost to the Air Force once
the applicant leaves active service. Therefore, the recommends the
applicant’s records be corrected as indicated below.

The pertinent military records of the Department of the Air Force relating
to APPLICANT, be corrected to show that Block 3 of DD Form 4/1,
Enlistment/Reenlistment Document Armed Forces of the United States, dated
12 March 2002, be amended to reflected Home of Record address as
