THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the:
* Army Achievement Medal (AAM) with 2nd Oak Leaf Cluster (OLC)
* Military Outstanding Volunteer Service Medal
* President's Volunteer Service Award (Gold Level)
2. The applicant states, in effect, these awards are not shown on his DD Form 214.
3. The applicant provides copies of the following:
* DA Form 638 (Recommendation for Award) for AAM with 2nd OLC
* AAM certificate
* DA Form 638 for Military Outstanding Volunteer Service Medal
* President's Volunteer Service Award certificate
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army (RA) on 20 November 2008. He served in military occupational specialty 74D (chemical operations specialist). He served in Afghanistan from 25 January through 27 November 2010. He reenlisted in the RA on 10 October 2010.
2. He provided copies of the following:
* DA Form 638 which recommended him for award of the AAM with 2nd OLC for meritorious service from 20 November 2008 through 17 January 2013; this award was approved by Permanent Orders Number (PO) 13-024-005 on 25 January 2013
* AAM certificate dated 24 January 2013
* DA Form 638 which recommended him for award of the Military Outstanding Volunteer Service Medal for exceptional volunteer service from 5 October 2011 through 7 September 2012; this award was approved by PO Number 13-346-001 on 19 December 2013
* President's Volunteer Service Award certificate issued in recognition and appreciation of his commitment to strengthening our nation and for making a difference through volunteer service in 2013
3. He was honorably discharged on 17 February 2013, by reason of a combat-related disability, with severance pay. He was credited with completing 4 years, 2 months, and 28 days of net active service. His DD Form 214 lists the:
* Afghanistan Campaign Medal with one bronze service star
* Army Commendation Medal
* AAM (2nd Award)
* Army Good Conduct Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Army Service Ribbon
* Overseas Service Ribbon
* North Atlantic Treaty Organization Medal
* Combat Action Badge
* Expert Marksmanship Qualification Badge with Machinegun Bar
4. Army Regulation 600-8-22 (Military Awards) does not state any provisions for listing citizen awards on the DD Form 214.
5. The Presidents Volunteer Service Award is the premier volunteer awards program, encouraging United States citizens or lawfully admitted permanent residents of the United States through presidential recognition to live a life of service. Any individual, family, or group that meets the programs criteria can receive Presidential recognition for volunteer hours served over a 12-month period or over the course of a lifetime. The award is issued in three levels: Silver, Bronze, and Gold. The award recipients can choose an official Presidents Volunteer Service Award pin, or a personalized certificate of achievement, or a congratulatory letter from the President of the United States, or combination of all three.
DISCUSSION AND CONCLUSIONS:
1. Orders awarded the applicant the AAM with 2nd OLC (3rd Award) and Military Outstanding Volunteer Service Medal during his period of service. Therefore, he is entitled to correction of his DD Form 214 to show these awards.
2. By regulation there are no provisions for listing citizen awards on the DD Form 214. Therefore, he is not entitled to correction to his DD Form 214 to show the President's Volunteer Service Award (Gold Level).

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to the applicant's DD Form 214 the Army Achievement Medal (3rd Award) in lieu of the Army Achievement Medal (2nd Award) and the Military Outstanding Volunteer Service Medal.
2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to correction of his DD Form 214 to show the President's Volunteer Service Medal (Gold Level).
