THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 26 April 2013 by:

	a.  amending item 12d (Total Prior Active Service) to show all her prior active service;

	b.  amending item 12f (Foreign Service) to show all her foreign service; and 

	c.  adding the Driver and Mechanic Badge with Driver – Wheeled Vehicle, three additional awards of the Armed Forces Reserve Medal with "M" Device, a third award of the Army Commendation Medal (ARCOM), and a second award of the Army Achievement Medal (AAM).  

2.  The applicant states:

	a.  she wants the information contained in her DD Form 214 for the period ending 21 January 2008 added to her DD Form 214 for the period ending 
26 April 2013.

	b.  the demobilization site did not have her prior DD Form 214 and was unable to add the information to her current DD Form 214.  She brought them all her prior orders and awards, but they still refused to add it to the DD Form 214 saying that only a DD Form 214 could give them proof that she deployed.   


3.  The applicant provides:

* DD Forms 214 for the periods ending 24 August 1996, 3 June 2004, 
      28 April 2006, 21 January 2008, and 26 April 2013
* Two DA Forms 638 (Recommendation for Award)   

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the U.S. Army Reserve on 25 January 1996 and she trained as a metal worker and later as a traffic management coordinator.  She was ordered to active duty on 7 July 2003 in support of Operation Enduring Freedom.  She served in Afghanistan from 12 August 2003 to 7 May 2004.  On 
3 June 2004, she was released from active duty (REFRAD).  Her DD Form 214 for the period ending 3 June 2004 shows, among other awards, the Armed Forces Reserve Medal with "M" Device.

2.  She was ordered to active duty on 25 September 2005 in support of Operation Iraqi Freedom.  She served Kuwait/Iraq from 10 October 2005 to 
2 April 2006.  On 28 April 2006, she was REFRAD.  Her DD Form 214 for the period ending 28 April 2006 shows, among other awards, the:

* ARCOM
* AAM
* Armed Forces Reserve Medal with "M" Device (2nd Award)   

3.  She was ordered to active duty on 15 September 2006 in support of Operation Iraqi Freedom.  She served Kuwait from 10 November 2006 to 
6 November 2007.  On 21 January 2008, she was REFRAD.

4.  Her DD Form 214 for the period ending 21 January 2008 shows in:

* Item 12c (Net Active Service This Period) the entry "0001 04 07" (1 year, 4 months, and 7 days)
* Item 12d the entry "0002 00 03" (2 years and 3 days)
* Item 12f the entry "0002 02 16" (2 years, 2 months, and 16 days)

5.  This DD Form 214 shows, among other awards, the:

* ARCOM (2nd Award)
* AAM
* Armed Forces Reserve Medal with "M" Device (3rd Award)
* Driver and Mechanic Badge with Driver – Wheeled Vehicle   
6.  Having completed a total of 3 years, 4 months, and 10 days of prior active service, she was ordered to active duty on 14 May 2012 in support of Operation Enduring Freedom.  She served Afghanistan from 18 September 2012 to 
16 March 2013.  

7.  She provided a DA Form 638, dated 18 January 2013, that recommended her for award of the ARCOM (2nd oak leaf cluster (OLC)), in effect, the ARCOM (3rd Award), for the period 9 April 2012 to 7 March 2013.  Item 8 (Previous Awards) of this form shows the entry "AAM-1" and "ARCOM-2" (two awards).

8.  The corresponding ARCOM certificate shows she was awarded the ARCOM for the period 9 April 2012 to 7 March 2013 by Permanent Orders Number 034-076, dated 3 February 2013.

9.  She also provides Permanent Orders Number 067-007, dated 8 March 2013, which show she received the AAM for the period 1 November 2012 to 6 March 2013.

10.  On 26 April 2013, she was REFRAD.  

11.  Her DD Form 214 for the period ending 26 April 2013 shows in:

* Item 12d the entry "0002 00 03" (2 years and 3 days)
* Item 12f the entry "0000 08 29" (8 months and 29 days)

12.  This DD Form 214 also shows the applicant was awarded or authorized the:

* Iraq Campaign Medal with two bronze service stars
* ARCOM (2nd Award)
* AAM
* Army Reserve Components Achievement Medal (3rd Award)
* National Defense Service Medal
* Afghanistan Campaign Medal with two bronze service stars
* Global War on Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* Army Service Ribbon 
* Armed Forces Reserve Medal with "M" Device
* North Atlantic Treaty Organization (NATO) Medal   

13.  Army Regulation 600-8-22 (Military Awards) states the “M” Device will be worn on the Armed Forces Reserve Medal to denote service by Reserve Component personnel who volunteered or were called to active duty during a mobilization or a contingency designated by the Secretary of Defense.  The regulation states when a member qualifies for the "M" Device, the Bronze "M" will be awarded, positioned on the ribbon and medal, and a number will be included on the ribbon and medal.  No more than one Armed Forces Reserve Medal may be awarded to any one person.  Multiple periods of service during one designated contingency will count as one "M" Device award.  The regulation also states the ten-year device is authorized for wear on the Armed Forces Reserve Medal to denote each 10-year period.  A bronze hourglass will be awarded upon completion of the first 10-year period award.

14.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  The regulation provides that for:

* item 12d – enter all active service less time lost
* item 12f – enter the total amount of foreign service completed during the period covered in item 12c (Net Active Service This Period) from the Soldier's record

DISCUSSION AND CONCLUSIONS:

1.  The applicant's request to amend item 12d of her DD Form 214 for the period ending 26 April 2013 to show all her prior active service was noted.  However, since item 12d includes annual training which would not show on a DD Form 214, it appears her entire records are not complete.  The U.S. Army Human Resources Command should review the applicant’s entire records and calculate her prior active service.  Any correction to item 12d should then be made, if necessary, to her DD Form 214 for the period ending 26 April 2013.

2.  Her request to amend item 12f of her DD Form 214 for the period ending 
26 April 2013 to show all her foreign service was noted.  However, since the governing regulation states the total amount of foreign service completed during the period covered by the DD Form 214 will be entered in item 12f, there is no basis for granting this request.

3.  The entry on her DD Form 214 for the period ending 21 January 2008 which shows entitlement to the Driver and Mechanic Badge with Driver – Wheeled Vehicle is accepted as sufficient evidence on which to base adding this badge to her DD Form 214 for the period ending 26 April 2013.

4.  Her request to show the Armed Forces Reserve Medal with "M" Device 
(4th Award) on her DD Form 214 for the period ending 26 April 2013 was noted.  However, the governing regulation states multiple periods of service during one designated contingency will count as one "M" Device award.  She was called to active duty on four separate occasions during one contingency – the Global War on Terror.  Therefore, the Armed Forces Reserve Medal with "M" Device is properly shown on her DD Form 214 for the period ending 26 April 2013 (although incorrectly shown on earlier DD Forms 214) and there is no basis for granting this request.  

5.  Based on her service in the USAR from 25 January 1996 through 24 January 2006, she is eligible for a bronze hourglass for wear on her Armed Forces Reserve Medal.  Her DD Form 214 for the period ending 26 April 2013 should be corrected to show this award.    

6.  Her DD Form 214 for the period ending 26 April 2013 shows two awards of the ARCOM.  However, the DA Form 638 provided by the applicant shows she had two previous awards of the ARCOM prior to her recommendation for the third award of the ARCOM for the period 9 April 2012 to 7 March 2013.  A corresponding ARCOM certificate shows Permanent Order Number 034-076, dated 3 February 2013, awarded her this ARCOM.  Therefore, there is sufficient evidence in which to base correcting her DD Form 214 for the period ending 
26 April 2013 to show three awards of the ARCOM.

7.  Her DD Form 214 for the period ending 26 April 2013 shows one award of the AAM, but the DA Form 638 provided by the applicant shows she had one previous award of the AAM prior to January 2013.  Permanent Orders Number 067-007, dated 8 March 2013, show she received the AAM for the period 
1 November 2012 to 6 March 2013.  Therefore, there is sufficient evidence in which to base correcting her DD Form 214 for the period ending 26 April 2013 to show two awards of the AAM.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the entries "ARMY COMMENDATION MEDAL (2ND AWARD)" and "ARMY ACHIEVEMENT MEDAL" from item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of her 26 April 2013 DD Form 214;

	b.  adding the Driver and Mechanic Badge with Driver – Wheeled Vehicle, Army Commendation Medal (3rd Award), and Army Achievement Medal (2nd Award) to item 13 of her 26 April 2013 DD Form 214; and  

	c.  having the U.S. Army Human Resources Command calculate all of her prior active service and amend items 12d and, if required, item 12e (Total Prior Inactive Service) of her 26 April 2013 DD Form 214.

2.  The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to adding three additional awards of the Armed Forces Reserve Medal with "M" Device to her DD Form 214 for the period ending 26 April 2013 or amending items 12f of this DD Form 214.
