THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Active Duty) to show the Army Commendation Medal (ARCOM) and Army Achievement Medal (AAM). 2. The applicant states she received the omitted ARCOM and AAM awards after the completion of his DD Form 214. 3. The applicant provides: * DD Form 214 * DA Form 638 (Recommendation for Award) dated 22 February and 10 May 2011 CONSIDERATION OF EVIDENCE: 1. The applicant's military record shows she was appointed a Reserve commissioned officer in the rank of second lieutenant (2LT) and swore her oath of office on 18 May 2007. 2. On 1 June 2007, she was ordered to active duty in the Regular Army. She was trained and served in area of concentration 25A (Signal). 3. The applicant's DA Form 4037 (Officer Record Brief) contains the "ARCOM-2" and "AAM-2" among the list of awards shown in Section VIII (Awards and Decorations). 4. On 1 June 2011, the applicant was honorably discharged from active duty after completing 4 years and 1 day service. Her DA Form 214 shows she earned the following awards: * Iraq Campaign Medal with two campaign stars * Army Commendation Medal * Army Achievement Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon (2nd Award) 5. The applicant provides the following two documents that show: a. DA Form 638 dated 22 February 2011 shows the applicant was awarded the ARCOM with Oak Leaf Cluster (OLC) and it was announced in Headquarters, 21st Theater Sustainment Command, Order Number 90-04, dated 30 March 2011. b. DA Form 638 dated 10 May 2011 shows the applicant was awarded the AAM with 1 OLC and it was announced in Headquarters, 16th Sustainment Brigade, Order Number 130-01, dated 10 May 2011. DISCUSSION AND CONCLUSIONS: The applicant contends her DD Form 214 should be corrected to show she received two ARCOMs and two AAMs was carefully considered and found to have merit. She provides a separate DA Form 638 that confirms the appropriate award authority approved her receipt of these awards and include the official orders announcing these awards. In addition her DA Form 4037 also confirms her entitlement to these awards. Therefore, it would be appropriate to correct her DD Form 214 to document these awards.
