THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his award of the Army Achievement Medal and Military Outstanding Volunteer Service Medal.

2.  The applicant states these two awards are missing from his DD Form 214 due to the receipt of the final copy of his DD Form 214 before they were approved.  

3.  The applicant provides copies of two DA Forms 638 (Recommendation for Award).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 11 May 2004.  He completed training and was awarded military occupational specialty 14G (air defense battle management system operator).  He served in Iraq from 4 March 2005 through 16 February 2006.

2.  He provided copies of the following:

   a.  A DA Form 638, dated 28 November 2012, recommending him for award of the Military Outstanding Volunteer Service Medal for exceptional volunteer service to the U.S. Army and Warrior Transition Battalion from 13 July 2012 through 9 October 2012.  The form shows the award was approved on 30 November 2012.
   
   b.  A DA Form 638, dated 29 November 2012, recommending him for award of the Army Achievement Medal with 4th Oak Leaf Cluster (OLC) (5th Award) for meritorious service from 25 July 2012 through 19 February 2013.  The form shows the award was approved on 25 January 2013, by Permanent Order (PO) Number 363-103.

3.  He served continuously on active duty through several reenlistments until he was discharged on 19 February 2013, by reason of a combat-related disability, with severance pay.  He was credited with completing 8 years, 9 months, and 9 days of net active service with no lost time.  His DD Form 214 lists the:

* Global War on Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* Iraq Campaign Medal with one bronze service star (2nd Award)
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Overseas Service Ribbon
* Combat Action Badge
* Driver and Mechanic Badge with Driver-Tracked Vehicle Clasp
* Driver and Mechanic Badge with Driver-Wheeled Vehicle Clasp

DISCUSSION AND CONCLUSIONS:

1.  PO awarded the applicant the Army Achievement Medal (5th Award) during his period of service.  Therefore, he is entitled to correction of his DD Form 214 to show this award.

2.  He was also awarded the Military Outstanding Volunteer Service Medal on 30 November 2012.  He is therefore entitled to correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from the applicant’s DD Form 214 the Army Achievement Medal (4th Award) and adding to his DD Form 214 the Army Achievement Medal (5th Award) and Military Outstanding Volunteer Service Medal.
