THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests removal of a duplicate Office of the Secretary of Defense (OSD) Identification Badge Certificate, dated 10 August 2011, and DA Form 705 (Army Physical Fitness Test Scorecard), dated 10 July 2010, from his Army Military Human Resource Record (AMHRR).

2.  The applicant states:

* the documents were erroneously added to his official records and should never have been submitted for filing
* there are duplicate OSD Identification Badge Certificates filed in his record and one should be removed
* a DA Form 705 is not part of his official records and should have never been filed in his AMHRR

3.  The applicant provides copies of his OSD Identification Badge Certificate and DA Form 705.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 17 August 1995.  He is currently serving in the Regular Army in pay grade E-7.

2.  A review of the interactive Personnel Electronic Records Management System (iPERMS) and the U.S. Army Human Resources Command Intergrated Web Services (IWS) Soldier Management System shows the applicant's DA Form 705 was filed therein on 13 October 2010.

3.  The OSD Identification Badge Certificate was initially filed in iPERMS and IWS on 13 September 2012.  It was filed a second time on 20 September 2012.

4.  Army Regulation 600-8-104 (Army Military Human Resource Records Management) provides policies, operating tasks, and steps governing the AMHRR.  Depending on the purpose, documents will be filed in the AMHRR in one of three folders:  performance, service, or restricted.  Once placed in the AMHRR, the document becomes a permanent part of that file and will not be removed or moved to another part of the AMHRR unless directed by appropriate authority.  Table 6-2 provides for the transfer of documents and states a DA Form 705 will be removed upon inprocessing and sent to the member's new unit commander.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contentions have been noted and his supporting evidence has been considered.

2.  According to iPERMS and IWS, the OSD Identification Badge Certificate was filed twice in his AMHRR and one copy should be removed.

3.  According to table 6-2 of Army Regulation 600-8-104, his DA Form 705 should have been forwarded to his new unit commander.  It should not have been filed in his AMHRR in iPERMS and IWS.

4.  In view of the foregoing, the applicant's records should be corrected as recommended below.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by removing the DA Form 705 filed on 13 October 2010 and the duplicate OSD Identification Badge Certificate filed on 20 September 2012 from his AMHRR in iPERMS and IWS.
