THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Meritorious Service Medal (4th Oak Leaf Cluster) and Armed Forces Reserve Medal.

2.  The applicant states he received the awards after his retirement.

3.  The applicant provides:

* DD Form 214
* memorandum authorizing him award of the Armed Forces Reserve Medal (1st Award)
* Permanent Orders (PO) Number 14-1, dated 14 January 1999, for the Meritorious Service Medal
* PO Number 45-2, dated 13 February 2002, and DA Form 638 for the Meritorious Service Medal
* PO Number 05-225, dated 13 August 2005, for the Meritorious Service Medal
* Orders 318-3, dated 14 November 2007, for the Meritorious Service Medal
* PO Number 152-036, dated 31 May 2012, and DA Form 638 (Recommendation for Award) for the Meritorious Service Medal with 4th Oak Leaf Cluster


CONSIDERATION OF EVIDENCE:

1.  The applicant's records show he enlisted in the U.S. Army Reserve (USAR) on 10 December 1988.  He served through multiple extensions or reenlistments.  He entered active duty on 12 May 2002.  He served in a variety of stateside assignments and he attained the rank/grade of master sergeant (MSG)/E-8.

2.  He retired on 31 August 2012 and he was placed on the Retired List in the rank/grade of MSG/E-8 on 1 September 2012.  His DD Form 214 shows he was awarded or authorized the:

* Meritorious Service Medal (4th Award)
* Army Commendation Medal (2nd Award)
* Army Achievement Medal (8th Award)
* Army Good Conduct Medal (7th Award)
* National Defense Service Medal (2nd Award)
* Global War on Terrorism Service Medal
* Noncommissioned Officer Professional Development Ribbon with Numeral 3
* Army Service Ribbon
* Overseas Service Ribbon
* Air Assault Badge
* Driver and Mechanic Badge with Driver-W Clasp
* German Armed Forces Badge-Bronze
* German Armed Forces Efficiency Badge-Gold
* Career Counselor Badge

3.  His records contain a memorandum from Headquarters, 8th Battalion, Army Reserve Careers Division, Fort Snelling, MN, dated 15 May 2012, authorizing him the Armed Forces Reserve Medal (1st Award) for service from 1 April 2002 to 31 March 2012.

4.  PO Number 14-1 issued by Headquarters, U.S. Army Test and Evaluation Command, Aberdeen Proving Ground, MD, dated 14 January 1999, awarded him the Meritorious Service Medal for meritorious service from 1 July 1993 to 17 December 1998.

5.  PO Order Number 45-2 issued by Headquarters, U.S. Army Combined Arms Support Command, Fort Lee, VA, dated 5 February 2002, awarded him the Meritorious Service Medal for meritorious service from 15 January 1999 to 15 February 2002.

6.  PO Number 05-225 issued by Headquarters, 84th Army Reserve Readiness Training Command, Fort McCoy, WI, dated 13 August 2005, awarded him the Meritorious Service Medal for meritorious service from 20 May 2002 to 28 August 2005.

7.  PO Number 318-3 issued by Headquarters, 99th Regional Readiness Command, Coraopolis, PA, dated 14 November 2007, awarded him the Meritorious Service Medal for meritorious service from 15 August 2005 to 9 December 2007.

8.  PO Number 152-036, issued by Headquarters, U.S. Army Reserve Command, Fort Bragg, NC, dated 31 May 2012, awarded him the Meritorious Service Medal for meritorious service (retirement) from 31 August 2002 to 30 August 2012.

9.  Army Regulation 600-8-22 (Military Awards) states the Armed Forces Reserve Medal is awarded for honorable and satisfactory service as a member of one or more of the Reserve Components for a period of 10 years.  The 10-Year Device (hourglass with Roman numeral "X") is authorized for wear on the Armed Forces Reserve Medal to denote each 10-year period of Reserve Component service.  The first 10-year period is denoted by a bronze hourglass, the second 10-year period by a silver hourglass, and the third 10-year period by a gold hourglass.

DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded five Meritorious Service Medals.  His DD Form 214 listed only four awards.  He is entitled to correction of his DD Form 214 to show five awards.

2.  The applicant was awarded the Armed Forces Reserve Medal (1st Award) for service from 1 April 2002 to 31 March 2012.  He is entitled to correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* deleting the Meritorious Service Medal (4th Award) from his DD Form 214
* adding the Meritorious Service Medal (5th Award) and Armed Forces Reserve Medal (1st Award) to his DD Form 214
