THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests removal of a duplicate copy of her Official Transcript of Academic Record from her official military personnel file (OMPF).
2. The applicant states she reviewed her file and found her OMPF contains two copies of her Official Transcript of Academic Record.
3. The applicant provides her Official Transcript of Academic Record and a certificate.
CONSIDERATION OF EVIDENCE:
1. Having prior service, the applicant's records show she enlisted in the Regular Army on 7 November 2007. She is assigned to Headquarters, 4th Infantry Division, Fort Carson, CO.
2. Her OMPF contains two copies of her Official Transcript of Academic Record issued on 12 January 2011.
3. Chapter 2 of Army Regulation 600-8-104 (Military Personnel Information Management/Records) governs the composition of the OMPF and states the performance section is used for filing performance, commendatory, and disciplinary data. Once placed in the OMPF, the document becomes a permanent part of that file. The document will not be removed from or moved to another part of the OMPF unless directed by certain agencies, to include this Board.
DISCUSSION AND CONCLUSIONS:
The evidence of record shows the applicant's OMPF contains two copies of her Official Transcript of Academic Record. Therefore, the applicant is entitled to correction of her records by removing the duplicate document.

* ensuring the copy of her Official Transcript of Academic Record she submitted with her application is not filed in her OMPF
