THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of Item 12a (Date entered AD This
Period) of her DD Form 214 (Certificate of Release or Discharge from Active
Duty) dated 23 January 2004 to show 20030902 instead of 20030708.
2. The applicant states, in effect, that the date she entered on initial
active duty for training (IADT) is not the date currently shown on her DD
Form 214.
3. The applicant provides a copy of Department of Defense Military
Entrance Processing Station, Des Plains, Illinois Orders Number 3245014
dated 2 September 2003, which amend Orders Number 3163-15 dated 17 June
2003 of the same headquarters. These orders amend the applicant’s IADT
will proceed date from 8 July 2003 to 2 September 2003. The applicant also
provides a copy of her Individual Clothing Record, which shows she received
her initial issue of military clothing at Fort Jackson, South Carolina on 9
September 2003.
CONSIDERATION OF EVIDENCE:
1. The applicant’s military records show she enlisted in the Illinois Army
National Guard (ILARNG) on 11 June 2003, for a period of 8 years, and was
initially ordered to IADT on 8 July 2003. She completed basic and advanced
individual training and was awarded military occupational specialty 92Y
(Unit Supply Specialist). The applicant returned to her unit in the ILARNG
after being released from IADT on 23 January 2004.
2. The applicant subsequently enlisted in the Regular Army on 3 June 2004
and is currently serving on active duty at Fort Knox, Kentucky in pay grade
E-3.
3. Item 12c (Net Active Service This Period) of the applicant’s DD Form
214 dated 23 January 2004 currently shows a total 6 months and 16 days.
DISCUSSION AND CONCLUSIONS:
1. Orders show the applicant’s will proceed date to IADT was amended from
8 July to 2 September 2003. Therefore she is entitled to correction of her
DD Form 214 dated 23 January 2004 to show 20030902.
2. As a result of changing Item 12a of her DD Form 214, Item 12c of the
form must also be changed to show the correct amount of active service
covered by the DD Form 214. Therefore, Item 12c should be corrected to
show a total net active service of 4 months and 22 days.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
correcting Item 12a of the individual’s DD Form 214 dated 23 January 2004
to show 20030902; and by correcting Item 12c to show 4 months and 22 days
of net active service during the period covered by the DD Form 214.
