Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the correct date she entered active duty, the net active service she completed, and her mission support to Operation Enduring Freedom (OEF). 2. The applicant states that when she was released from active duty, she did not have copies of her orders which led to her DD Form 214 listing an incorrect entry date onto active duty and incorrect net active service. She adds that her OEF mission support is also not shown on her DD Form 214. 3. The applicant provides the following additional documentary evidence in support of her application: a. DD Form 214, dated 13 August 2008. b. Orders 07-120-00397 and Orders 08-074-00065, issued by 330th Medical Brigade, Fort Sheridan, Illinois, on 30 April 2007 and 14 March 2008. c. Orders 032861, issued by the Army Reserve Medical Command, Pinellas Park, Florida, on 25 May 2007. d. Permanent Orders 235-006 and 169-188, issued by the U.S. Army Installation Management Command (IMCOM), U.S. Army Garrison (USAG), Fort McCoy, Wisconsin, on 23 August 2007 and 17 June 2008. CONSIDERATION OF EVIDENCE: 1. With prior enlisted service, the applicant's records show she was appointed as a second lieutenant in the Army Nurse Corps (ANC) of the U.S. Army Reserve (USAR) and executed an oath of office on 6 June 1990. She subsequently served in various staff and leadership positions and was promoted to captain on 25 June 1998 and major on 21 July 2003. 2. On 30 April 2007, while assigned to the 325th Medical Hospital, Independence, Missouri, her higher headquarters, the 330th Medical Brigade, Fort Sheridan, Illinois, published orders 07-120-00397, ordering the applicant to active duty as a member of her Reserve unit for a period of 400 days in support of Operation Iraqi Freedom (OIF). She was ordered to report to Fort McCoy, Wisconsin, effective 23 June 2007. 3. On 25 May 2007, the Army Reserve Medical Command, Pinellas Park, Florida, published orders 032861, ordering the applicant to active duty training (ADT) for the purpose of OIF/OEF mission support sustainment training. She was ordered to report to Fort McCoy, Wisconsin, for a period of 22 days effective 1 June 2007 and to return to the place where she entered active duty upon completion of the period of ADT so she can be relieved from ADT. 4. On 23 August 2007, Headquarters, USAG, IMCOM, Fort McCoy, Wisconsin, published Permanent Orders 235-006 ordering the 325th Medical Company, Combat Support Hospital, on a temporary change of station (TCS) and assigning the unit to Headquarters, U.S. Army Central Command, Bagram, Camp Doha, Kuwait, effective on or about 24 August 2007. 5. On 17 June 2008, Headquarters, USAG, IMCOM, Fort McCoy, Wisconsin, published Orders 169-188, ordering the applicant's release from active duty to the control of her USAR unit effective 13 August 2008. Accordingly, she was honorably released from active duty and transferred to her USAR unit on 13 August 2008. 6. The DD Form 214 the applicant was issued at the time of her release from active duty shows the following entries: a. Item 12a (Date Entered Active Duty This Period) shows the entry "2007  06  20"; b. Item 12b (Separation Date This Period) shows the entry " 2008  08  13"; c. Item 12c (Net Active Service This Period) shows the entry "0001  01  24"; and d. Item 18 (Remarks) shows the applicant was ordered to active duty in support of OIF in accordance with section 12302, Title 10, U.S. Code, and that she served in a designated imminent danger pay area/served in Iraq from 26 August 2007 to 12 June 2008. 7. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of their military service. It is important that information entered on the form should be complete and accurate. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. Item 12 shows the Record of Service. Extreme care is used when completing this block since post-service benefits, final pay, retirement credit, and so forth are based on this information. Item 12a shows the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued. Item 12b shows the Soldier’s transition date. This date may not be the contractual date if Soldier is separated early, voluntarily extends, or is extended for make-up of lost time, or retained on active duty for the convenience of the Government. Item 12c shows the amount of service this period, computed by subtracting item 12a from 12b. Lost time under 10 USC 972 and non-creditable time after expiration term of service, if any, are deducted and such time will be identified in block 18. Item 18 of the Soldier's DD Form 214 is used for mandatory requirements when a separate block is not available. For an active duty Soldier deployed with his or her unit during their continuous period of active service, enter the statement "Service in (Name of County Deployed) from (inclusive dates for example, YYYYMMDD – YYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. The applicant contends that her DD Form 214 should be corrected to show the correct date she entered active duty, the net active service she completed, and her mission support to Operation Enduring Freedom (OEF). 2. With respect to the applicant's entry date, the evidence of records shows that she was ordered to ADT for a period of 22 days, no later than 1 June 2007, for OIF/OEF mission support/sustainment training. Her orders stipulated that she should return to the place where she entered ADT to be relieved from such duty. It is unclear if the applicant entered ADT as instructed and/or returned to the place where she entered ADT for release. 3. The applicant's DD Form 214 shows she entered active duty on 20 June 2007, served in Iraq from 26 August 2007 to 12 June 2008, and was honorably released from active duty on 13 August 2008. She completed 1 year, 1 month, and 24 days of active federal service during this period of active duty. There is no evidence in the applicant's records and the applicant did not provide any evidence that shows these entries are incorrect. Absent evidence to the contrary, it must be presumed that these dates are correct. 4. With respect to the applicant's OEF mission support, there is no provision to enter this period of sustainment training on her DD Form 214.
