	The Board considered the following evidence: 

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 12a (Date Entered AD This Period) of her DD Form 214 (Certificate of Release or Discharge from Active Duty). 

2.  The applicant states, in effect that she entered active duty on 8 July 2003 rather than 23 September 2003.  

3.  The applicant provides a copy of her DD Form 214.  She indicates, “Copy of corrective orders attached.” 

CONSIDERATION OF EVIDENCE:

1.  The applicant joined the Vermont Army National Guard on 15 November 2002.  Orders number 226-3 issued by the Military Entrance Processing Station (MEPS) Springfield, Massachusetts directed her to report for initial active duty for training (IADT) on 3 September 2003.    

2.  Military Entrance Processing Station (MEPS) Springfield, Massachusetts orders number 3156, dated 5 June 2003, modified the previous IADT orders.  She was to report on 6 July 2003 vice 3 September 2003.   

3.  Her DD Form 214 shows that she entered initial active duty for training on 23 September 2003.  On 16 October 2003, she was released from active duty training and discharged as a Reserve of the Army due to failure to meet procurement medical standards.

4.  An MGB Form 23A (Army National Guard Current Annual Statement), dated 30 December 2003, was issued by the Vermont National Guard.  It credited her with 101 days of active duty commencing on 8 July 2003 and ending on 16 October 2003.   

DISCUSSION AND CONCLUSIONS:

1.  The available evidence shows that the applicant entered active duty on 6 July 2003.   

2.  Item 12a of the DD Form 214 should be corrected to read "2003 07 06."  The resultant Net Active Service This Period (Item 12c) should be changed to read "0000 03 11."

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by changing Item 12a of the DD Form 214 to read "2003 07 06" and changing Item 12c to read "0000 03 11."
