school  and  j o i n   the  Airforce.  I was  a  good  Airman  and  a 

excellent  police o f f i c e r .   I d i d  not  comitt  a  crime.  ~ e i n g

the  ~l k i n s  Mountain  School,  t r y i n g   t o  help  children  t u r n  

t h i e r   l i v e s  around,  and  he1 p  them  make  something  out  o f  

t h i e r   l i v e s ,   maybe  even  j o i n   the  ~ i l i t a r y  i f  they  wanted. 

I enjoy  being envolved  i n  Law  Enforcement  work. 

And  one  day  would  1 i ke  t o  make  a  carrer  as  a  police 

o f f i c e r .   MY  A i  rforce  EPR'S  r e f l e c t  my  excel l e n t  service

I hold  no  ill w i l l   against  anyone  i n  the 

Airforce.  As  a  yourlg  Airman  I made  a  mistake.  I was  a 

great ~i rman  and  would  l i k e  t o  be  one  again.

1.  I,  am recommending your discharge from the United States Air Force for a pattern of 
misconduct.  The authorities for this action are AFPD 36-32 and AFI 36-3208, paragraph 550.2. 
If my recommendation is approved, your service may be characterized as honorable or under 
honorable conditions (general).  I am recommending that your service be characterid as under 
honorable conditions (general). 

2. My reasons for this action are: 

a.  On 23 Nov 01, you failed to obey 
your First Sergeant, by personally co 
incident you received a Letter of Reprimand, dated 7 Dec 01.  (Atch 1 a) 

b.  Between 29 Sep 01 and 19 Oct 01, you failed to obey a lawful 
was issued to you by your First Sergeant, by personally contacting

c.  On 8 Jun 01, you missed a scheduled dental appointment.  For this incident you 
received a Letter of Counseling, dated 27 Jun 01.  (Atch 1 c) 

d.  On 18 Jan 00, you failed to provide information you possessed regarding the  location 
of a missing piece of gold jewelry.  For this incident you received an Article IS, dated 5 
Apr 00.  (Atch Id) 

3.  Copies of the documents to be forwarded to the separation authority in support of this 
recommendation are attached.  The commander exercising Special Court-Martial convening 
authority or a higher authority will decide whether you will be discharged or retained in the Air 
Force and, if you are discharged, how your service will be characterid.  If you are discharged 
you will be ineligible for reenlistment in the Air Force and any special pay, bonus or education 
assistance funds may be subject to recoupment.
