APPLICANT REQUESTS THAT: 

 

His DD Form 214, Armed Forces of the United States Report of 
Transfer or Discharge, be corrected to reflect his award of the 
Air Force Commendation Medal (AFCM).

APPLICANT CONTENDS THAT: 

 

He believes the omission of the AFCM on his DD Form 214 is a 
clerical error. 

 

In support of his application, the applicant provides copies of 
his DD Form 214, Presentation of Approved Decoration letter, AFCM 
Certificate and the AFCM Citation. 

 

The applicant’s complete submission, with attachments, is at 
Exhibit A.

STATEMENT OF FACTS: 

 

The applicant is a former member of the Regular Air Force who 
served on active duty from 12 September 1967 to 11 June 1971. He 
was progressively promoted to the grade of sergeant (E-4) with a 
date of rank of 1 June 1969. On 11 June 1971, he was honorably 
discharged from active duty and transferred to the Air Force 
Reserve.

AIR FORCE EVALUATION: 

 

AFPC/DPSIDR recommends denial. DPSIDR states that after a 
thorough review of the applicant’s official military record, they 
were unable to produce a copy of the special order authorizing 


him the AFCM; therefore, they must recommend disapproval of his 
request.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the applicant 
on 9 April 2010 for review and response within 30 days (Exhibit 
C). 

 

In response the applicant forwarded a copy of his AFCM Special 
Order #G-43, dated 13 May 1971 (Exhibit D).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by existing 
law or regulations. 

 

2. The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice. We note the 
applicant provided documentation (citation to accompany the award 
of the AFCM for the period 3 June 1970 to 3 June 1971 along with 
a copy of the certificate for the same award) to substantiate his 
claim. However, the Air Force office of primary responsibility 
indicates they are unable to administratively correct the 
applicant’s record to reflect his award of the AFCM without a 
copy of the special order, which authenticates the decoration. 
In response to the Air Force advisory opinion, the applicant 
provided a copy of Special Order G-43, dated 13 May 1971, as 
proof he was awarded the AFCM. Therefore, we recommend the 
records be corrected as indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air Force 
relating to APPLICANT be corrected to show that he was awarded 
the Air Force Commendation Medal, for meritorious service during 
the period 3 June 1970 to 3 June 1971.
