APPLICANT REQUESTS THAT:
He be awarded the Air Force Achievement Medal with one oak leaf cluster
(AFAM w/1OLC).

APPLICANT CONTENDS THAT:
He does not know why the contested award was not added to his DD Form 214,
Certificate of Release or Discharge from Active Duty.
In support of his request, the applicant provided a copy of his DD Form 214
and an unsigned copy of a certificate for award of the AFAM, w/1OLC.

STATEMENT OF FACTS:
On 8 April 1988, the applicant entered active duty in the Regular Air Force
and on 16 December 1992, he was honorably discharged in the grade of airman
first class, under the provisions of AFR 39-10, Exceeding Air Force Weight
Standards. He served four years, eight months, and eight days of total
active service.

AIR FORCE EVALUATION:
AFPC/DPPPR recommends denial for award of the AFAM w/1OLC. DPPPR states
the applicant’s official military personnel record did not contain a
special order awarding him the AFAM w/1OLC. DPPPR was able to locate the
certificate, but it was incomplete. The applicant was unable to furnish
the special order awarding him the AFAM w/1OLC.
After a thorough review of the applicant’s military record DPPPR did locate
an AF Form 104, Awards Verification, awarding the applicant the Southwest
Asia Service Medal with two bronze service stars (SWASM w/2BSS) and Kuwait
Liberation Medal for the period in question. The applicant’s DD Form 214
does reflect the aforementioned awards and will be updated to include the
KLM-SA.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
On 8 September 2006, the evaluation was forwarded to the applicant for
review and comment within 30 days (Exhibit D). As of this date, this
office has received no response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was not timely filed; however, it is in the interest
of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to demonstrate the
existence of an error or injustice. The applicant’s contentions are duly
noted; however, after reviewing the evidence of record, along with the
applicant’s submission, the Board agrees with the opinion and
recommendation of the Air Force and adopts its rationale as the basis for
their conclusion that the applicant has not been the victim of an error or
injustice. The Board notes evidence has not been provided and there is no
documentation in the applicant’s military personnel record to confirm that
he was awarded the AFAM w/1OLC. Therefore, in the absence of evidence to
the contrary, the Board finds no basis to recommend granting the relief
sought.

THE BOARD DETERMINES THAT:
The applicant be notified the evidence presented did not demonstrate the
existence of an error or an injustice; the application was denied without a
personal appearance; and the application will only be reconsidered upon the
submission of newly discovered relevant evidence not considered with this
application.
