APPLICANT REQUESTS THAT:
Her duty title on her P0505A Promotion Recommendation Form (PRF) and on her
Officer Selection Brief (OSB) previously reviewed by the CY05A Lieutenant
Colonel Selection Board (CSB) be corrected to read Element Chief, Women’s
Health Clinic (ECWHC) effective 15 March 2005, and the corrected record be
considered by a Special Selection Board (SSB) for promotion.

APPLICANT CONTENDS THAT:
The job titles reflected on her PRF and on her Single Uniform Retrieval
Format (SURF) were incorrect. The title shown was Women’s Health Nurse
Practitioner (WHNP); however, the correct duty title of Element Chief,
Women’s Health Clinic (ECWHC), effective March 15, 2005, was not indicated
as her duty title at the time the promotion board met. Her Jan 05 official
personnel brief correctly stated her job title. Her PRF was completed
prior to her movement to the element chief position. She worked closely
with her chain of command and local personnel office to ensure the job
change was reflected in her official personnel record. She received
assurance from all that the record was updated; however, upon review of her
record that met the board it was discovered that the duty title was not
correct as she had been informed.
In support of the application, she submits a copy of her personnel change
request, two support letters, and copies of her OSB and SURF. Her complete
submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:
Information extracted from the Military Personnel Data System (MilPDS)
indicates the applicant’s Total Active Federal Military Service Date as 7
June 1989. She is currently serving as a permanent party Air National
Guard officer in the grade of major, with a date of rank of 1 August 2001.
The applicant has one nonselection for promotion by the CY05A (6 Jul 05)
Lieutenant Colonel Central Selection Board (CSB).
The following is a resume of the top eleven (11) reports of her Officer
Selection Record commencing with the report closing 28 Oct 96.

the applicant’s military records, are contained in the letter prepared by
the appropriate office of the Air Force at Exhibit C.

AIR FORCE EVALUATION:
HQ AFPC/DPAMN recommends denial of the applicant’s request. DPAMN states
it is the member’s responsibility to ensure that records and all
information on the OSB is correct and accurate prior to the board date.
DPMAN notes her duty title was changed in Mar 05 and the board met on 5 Jul
05; therefore, the applicant had more than three months to ensure her
record was accurate and complete. DPAMN’s evaluation is at Exhibit C.
HQ AFPC/DPPPE recommends denial of the applicant’s request. DPPPE states
the Officer Preselection Briefs (OPBs) were forwarded to the Military
Personnel Flight (MPF) on 14 Feb 05 and the MPF suspense to disperse the
OPBs was 8 Mar 05. DPPPE notes the PRF cutoff date, which is the earliest
date senior raters can sign the PRF, was 6 May 05. Based on this
information, the PRF was not accomplished until after the applicant’s
movement to the element chief position. On or about 6 Jun 05 (30 days
prior to the board), the applicant would have received her PRF to review
for accuracy. She knew the PRF contained the wrong duty title but did not
take the necessary actions to correct the PRF. In addition, she should
have reviewed her records and surf information prior to the board convening
date of 6 Jul 05 to ensure her records were correct. DPPPE opines even
with asking the MPF if the update was made, she should have requested a
copy of her OPB or SURF prior to the board convening to verify her
information was accurate for the upcoming promotion board.
DPPPE notes if the applicant’s effective date for the duty title “Elem.
Chief, Women’s Health/WHPN” is 15 Mar 05, then the duty title on the OPR
closing out on 22 Mar 05 should be corrected. However, the evaluators (who
knew about the duty title change according to the applicant) signed the OPR
on 15 Apr 05 and left the duty title as “Women’s Health Nurse Practitioner
(NP). DPPPE explains this may be due to the fact the duty title effective
date (according to the most recent surf) show an effective date of 25 Apr
05, and if this is correct, the duty title on the OPR is correct. DPPPE
concludes the applicant still had time to review her PRF and request
changes prior to the board if her duty title change was effective 25 Apr
05.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force Evaluation was forwarded to the applicant for
review and comment on 19 May 2006. As of this date, this office has
received no response (Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to demonstrate the
existence of probable error or injustice. The applicant’s contentions are
duly noted; however, other than her own assertions, we have seen no
evidence by the applicant that would lead us to believe that the duty title
on her PRF and OSB, during the period in question, should be changed.
Therefore, we agree with the opinion and recommendation of HQ AFPC/DPPPE
and adopt their rational as the basis for our decision to not favorably
consider the applicant’s requests related to her PRF and OSB reviewed by
the CY05A Lieutenant Colonel Selection Board. Since we are unable to
conclude the applicant’s record, as seen by the CY05A selection board was
improperly constituted, the applicant’s request for SSB consideration is
denied.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not demonstrate
the existence of probable material error or injustice; that the application
was denied without a personal appearance; and that the application will
only be reconsidered upon the submission of newly discovered relevant
evidence not considered with this application.
