THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 19 August 2012, to show his Army Commendation Medal (2nd Award) and Army Good Conduct Medal (2nd Award).
2. The applicant states he received both awards while on terminal leave and was unable to have his Transition Center update his DD Form 214 before his expiration term of service date.
3. The applicant provides:
* Army Commendation Medal Certificate showing Permanent Orders (PO) Number 136-002, dated 15 May 2012, issued by Headquarters, 130th Engineer Brigade
* PO Number 213-003, dated 31 July 2012, issued by Headquarters, 84th Engineer Battalion (Construction Effects)
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 20 July 2006.
2. He was honorably released from active duty on 19 August 2012. His DD Form 214 shows he completed 6 years and 1 month of net active creditable service. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) lists the following:
* Army Commendation Medal
* Army Achievement Medal
* Meritorious Unit Commendation
* Army Good Conduct Medal (1st Award)
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Iraq Campaign Medal with one bronze service star
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Overseas Service Ribbon (2nd Award)
* Army Lapel Button
3. His records contain an Army Commendation Medal Certificate showing this award was issued in PO Number 357-003, dated 22 December 2008, by the 49th Group.
4. The applicant provides:
a. Army Commendation Medal Certificate showing PO Number 136-002, dated 15 May 2012, issued by Headquarters, 130th Engineer Brigade, reflecting a second award of the Army Commendation Medal.
b. PO Number 213-003, dated 31 July 2012, issued by Headquarters, 84th Engineer Battalion (Construction Effects) which shows the second award of the Army Good Conduct Medal for the period 20 July 2009 to 19 July 2012.
DISCUSSION AND CONCLUSIONS:
Permanent Orders show the applicant was awarded two Army Commendation Medals and two Army Good Conduct Medals. His DD Form 214 only shows one award of each. His DD Form 214 should therefore be corrected to show the Army Commendation Medal (2nd Award) and Army Good Conduct Medal (2nd Award).

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 for the period ending 19 August 2012 by:
a. deleting the Army Commendation Medal and Army Good Conduct Medal; and
b. adding the Army Commendation Medal (2nd Award) and Army Good Conduct Medal (2nd Award).
