THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show she was awarded the Army Good Conduct Medal (1st Award) and the Army Commendation Medal (2nd Award). 2. The applicant states she received the requested awards after she had completed her final outprocessing for separation; therefore, the requested awards were not listed on her DD Form 214. 3. The applicant provides: * Permanent Orders 222-002, issued by the 2nd Brigade Special Troops Battalion, 2nd Brigade Combat Team (BCT) and 502nd Parachute Infantry Regiment, 101st Airborne Division (Air Assault), Fort Campbell, KY, dated 9 August 2012 * DA Form 4950 (Good Conduct Medal Certificate), dated 9 August 2012 * DA Form 638 (Recommendation for Award), dated 13 August 2012, finalized as Permanent Order Number 228-001, issued by Headquarters, 2nd BCT, Fort Campbell, dated 21 August 2012 * DA Form 4980-14 (Army Commendation Medal Certificate), dated 15 August 2012 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 5 August 2009. She completed initial entry training and she was awarded military occupational specialty 42A (Human Resources Specialist). On 1 November 2010, she was promoted to the rank/grade of specialist (SPC)/E-4. 2. Her Enlisted Record Brief, dated 14 May 2012, shows she deployed to Afghanistan in support of Operation Enduring Freedom from 18 July 2010 to 17 April 2011. Additionally, this record shows she was previously awarded the Army Commendation Medal. 3. Permanent Orders 222-002, dated 9 August 2012, awarded her the Army Good Conduct Medal (1st Award) for the period 5 August 2009 through 5 August 2012. 4. Permanent Order Number 228-001, dated 21 August 2012, awarded her the Army Commendation Medal (2nd Award), for exemplary service during the period 1 January 2010 to 15 August 2012. 5. On 1 December 2012, she was honorably released from active duty and she was transferred to the U.S. Army Reserve (USAR) Control Group (Reinforcement) to complete her remaining service obligation. She completed 3 years, 3 months, and 27 days of creditable active service. The DD Form 214 she was issued at the time does not show award of the Army Good Conduct Medal. It does show one award of the Army Commendation Medal. DISCUSSION AND CONCLUSIONS: Permanent Orders awarded her the Army Good Conduct Medal (1st Award) and the Army Commendation Medal (2nd Award), which are not shown on her DD Form 214. Therefore, her DD Form 214 should be corrected to show these awards.
