* DA Form 67-9 (Officer Evaluation Report) covering the period 24 October 2012 through 23 October 2013
* DD Form 214 (Certificate of Release or Discharge from Active Duty)
* DD Form 214C (Continuation Sheet)
CONSIDERATION OF EVIDENCE:
1. The applicant was ordered to active duty as a member of the U.S. Army Reserve Control Group (Reinforcement) in support of Operation Enduring Freedom on 23 November 2012.
2. Records show he served as the officer in charge of the Justice Center in Parwan, NATO Rule of Law Field Support Mission, Rule of Law Field Force-Afghanistan, while he was deployed which fell under Headquarters, Combined Joint Interagency Task Force 435.

4. Joint Staff Permanent Order Number J-1SO-0261-13, dated 11 December 2013, announced award of the JMUA to Headquarters, Combined Joint Interagency Task Force 435, for exceptionally meritorious service during the period 1 November 2012 to 31 October 2013. The order specifies this award is for the headquarters unit only. Service units and individuals assigned to Service units in support of this organization are not eligible for this award. This permanent order was uploaded to the applicant's interactive Personnel Electronic Records Management System file.
5. Army Regulation 635-5 (Separations Documents) at the time prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for preparation of the DD Form 214. It states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge.
DISCUSSION AND CONCLUSIONS:
The evidence shows Headquarters, Combined Joint Interagency Task Force 435, was awarded the JMUA for service during a period the applicant was assigned to this unit. However, the award was announced after his DD Form 214 had been prepared and issued. Therefore, it would be appropriate to correct his DD Form 214 at this time by adding award of the JMUA.
