THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show completion of the Senior Leaders Course (SLC) and award of the Meritorious Service Medal (MSM).
2. The applicant states the:
* SLC wasn't added to his DD Form 214 during transition for separation
* MSM was not documented on his Enlisted Record Brief (ERB) to be added to his DD Form 214
3. The applicant provides:
* DA Form 638 (Recommendation for Award)
* MSM Certificate
* DA Form 1059 (Service School Academic Evaluation Report)
* Advanced Noncommissioned Officer Course (currently known as the SLC) Diploma
* ERB
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 10 September 1991 and served as an automated logistical specialist. He remained on active duty through continuous reenlistments until he retired on 31 January 2013 in the rank of staff sergeant.
2. Item 14 (Military Education) of his DD Form 214 is completed on a continuation sheet and clearly shows he completed the 6-week SLC in 2008. Since this training is shown on his DD Form 214, this portion of his request will not be discussed further in this Record of Proceedings. A copy of the continuation sheet will be provided to the applicant.
3. His DD Form 214 does not show the MSM as an authorized award.
4. He provided a DA Form 638 that shows he was awarded the MSM for exceptionally meritorious service during the period 1 January 2003 to 31 January 2013 by Headquarters, 1st Armored Division, Permanent Orders Number 298-01, dated 19 November 2012, and his corresponding MSM Certificate, dated 24 October 2012.
DISCUSSION AND CONCLUSIONS:
Permanent orders show the applicant was awarded the MSM. Therefore, his DD Form 214 should be corrected to show this medal.
