THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that a second Meritorious Service Medal (MSM) be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states the MSM was awarded after his discharge and as a result it is not included on his DD Form 214.

3.  The applicant provided a copy of his:

* DD Form 214
* DA Form 638 (Recommendation for Award)
* DA Form 4980-12 (MSM Certificate)
* Enlisted Record Brief (ERB)

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 27 February 2002.

2.  He submitted a: 

	a.  copy of his ERB, dated 3 March 2014, which shows in Section VIII (Awards and Decoration) the entry MSM (1st Award).

	b.  copy of a DA Form 638 which shows he was recommended for award of the MSM with 1st oak leaf cluster (2nd award) under the authority of Permanent Order Number 283-04, for exceptionally meritorious service from 28 October 2011 to 1 March 2014.

	c.  DA Form 4980-12, dated 10 October 2013, which shows that he was awarded the MSM for exceptionally meritorious service by Permanent Orders 283-04, Headquarters, First Army Division East, for meritorious service from 
28 October 2011 to 1 March 2014.

3.  On 1 March 2014, he was honorably discharged.  His DD Form 214 only shows one MSM.

DISCUSSION AND CONCLUSIONS:

The evidence of record shows that the applicant was awarded the MSM (2nd award).  Therefore, the applicant's records should be corrected to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by 
deleting from his DD Form 214 the Meritorious Service Medal and adding the Meritorious Service Medal (2nd award).
