THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show an additional award of the Meritorious Service Medal (MSM).
2. She states an award of the MSM was signed after her retirement date, which is the reason it was not included on her DD Form 214.
3. The applicant provides an MSM Certificate and a DA Form 638 (Recommendation for Award).
CONSIDERATION OF EVIDENCE:
1. With prior service in the U.S. Navy, on 10 September 1997, the applicant enlisted in the Regular Army (RA). She continued her RA service until she retired.
2. Her Army Military Human Resource Record contains two MSM Certificates. The certificates show Headquarters, U.S. Army John F. Kennedy Special Warfare Center and School issued:
* Permanent Orders (PO) Number 201-8, dated 20 July 2005, announcing award of the MSM to her for the period 10 June 2001 to 10 June 2005
* PO 058-04, dated 27 February 2008, announcing award of the MSM to her for the period 1 August 2006 to 1 January 2008
3. On 13 February 2012, she was retired for permanent disability. Her DD Form 214 shows she was awarded the MSM (2nd Award).
4. She provides an MSM Certificate and DA Form 638 showing Headquarters, U.S. Army Special Operations Command, issued PO 011-04, dated 11 January 2012, announcing award of the MSM to her for the period 1 February 2002 to
18 January 2012.
DISCUSSION AND CONCLUSIONS:
Shortly before the applicant's retirement, PO announced award of a third MSM to the applicant. This award is not shown on her DD Form 214. She is entitled to correction of her DD Form 214 to show the MSM (3rd Award).

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the MSM (2nd Award) from her DD Form 214 and adding the MSM (3rd Award).

The DA Form 638 that recommended her for award of the MSM does not show she was awarded an interim award (i.e., the ARCOM) and, in fact, shows that she had previously been awarded the ARCOM (2nd Award).
