THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) ending 18 February 2014 to show:
a. He was awarded the Bronze Star Medal, Army Achievement Medal, and the North Atlantic Treaty Organization (NATO) Medal.
b. He completed the Military Intelligence Officer Basic Leadership Course.
2. The applicant states that the Bronze Star Medal, Army Achievement Medal, and the NATO Medal were awarded after his DD Form 214 was completed. He completed the Military Intelligence Officer Leadership Course during a earlier period of active duty, but that training should also be shown on the DD Form 214 ending 18 February 2014.
3. The applicant provides copies of

CONSIDERATION OF EVIDENCE:
1. The applicant was appointed as a U.S. Army Reserve military intelligence second lieutenant (2LT) on 7 May 2007. He was promoted to first lieutenant (1LT) on 21 November 2012.
2. A DA Form 1059 shows he completed the Military Intelligence Officer Leadership Course on 12 March 2012.
3. A DA Form 638 shows he was awarded the Army Achievement Medal on
12 March 2012 for achievement from 26 October 2011 to 14 March 2012.
4. A DD Form 214 shows he served on active duty training from 26 October 2011 to 12 March 2012. This DD Form 214 shows in:
a. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the National Defense Service Medal and the Army Service Ribbon.
b. Item 14 (Military Education) the Military Intelligence Officer Leadership Course, 16 weeks, 2012.
5. He last served on active duty from 5 July 2013 to 18 February 2014 when he was honorably released from active duty. His DD Form 214 for that period of service shows in item 13 the
* Afghanistan Campaign Medal with one bronze service star
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Army Service Ribbon
* Armed Forces Reserve Medal with "M" Device
* German Armed Forces Proficiency Badge Gold
6. A DA Form 638 and certificate show he was awarded the Bronze Star Medal on 14 March 2014 for exceptionally meritorious service from 12 July 2013 to
20 January 2014.
7. A certificate shows he was awarded the NATO Medal for the period 17 July 2013 to 25 January 2014.
8. Army Regulation 635-5 (Separation Documents), in effect at the time, states in paragraph 2-1 that the DD Form 214 is a summary of a Soldiers most recent period of continuous active duty. It provides a brief, clear cut record of active duty service at the time of release from active duty, retirement, or discharge. For item 13, list awards and decorations for all periods of service (emphasis added) in the priority sequence specified in Army Regulation 600-8-22. For item 14 list in-service training courses; title, number of weeks, year successfully completed during the period of service covered by the DD Form 214 (emphasis added).
DISCUSSION AND CONCLUSIONS:
1. The available evidence shows he was awarded the Bronze Star Medal, Army Achievement Medal, and the NATO Medal. These awards should be added to his DD Form 214 ending 18 February 2014.
2. He completed the Military Intelligence Officer Leadership Course on 12 March 2012. That training is properly listed on his DD Form 214 ending 12 March 2012. It should not be listed on his DD Form 214 ending 18 February 2014.

1. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by

* Army Achievement Medal
* NATO Medal
2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to correcting his DD Form 214 ending 18 February 2014 to show he completed the Military Intelligence Officer Leadership Course in 2012.
