THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show a third award of the Army Commendation Medal (ARCOM).
2. The applicant states she received the ARCOM at home after completion of her DD Form 214.
3. The applicant provides:

* DA Form 638 (Recommendation for Award), dated 15 November 2012
CONSIDERATION OF EVIDENCE:
1. The applicant was ordered to active duty as a member of the U.S. Army Reserve on 5 January 2012 in support of Operation Enduring Freedom. On 3 January 2013, she was honorably released from active duty after completing 11 months and 29 days of active service.
2. Her DD Form 214 for this period of service shows award of the ARCOM (2nd Award).
3. She provides a DA Form 638 which shows Headquarters, 88th Regional Support Command, Permanent Order Number 12-3556-009, dated 21 December 2012, awarded her the ARCOM with 2nd Oak Leaf Cluster (denoting the third award) for meritorious service during the period 9 July 2011 to 1 December 2012.
DISCUSSION AND CONCLUSIONS:
The applicant's contention that her DD Form 214 should be corrected to show she was awarded a third ARCOM was carefully considered and found to have merit. She provides a DA Form 638 that shows she was awarded the ARCOM with 2nd Oak Leaf Cluster during the period covered by her DD Form 214. Therefore, it would be appropriate to correct her DD Form 214 to document this award.
