THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show a third award of the Army Achievement Medal (AAM).
2. The applicant states, in effect, his DD Form 214 does not reflect the third award of the AAM that he received as an expiration term of service (ETS) award.
3. The applicant provides:

CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 6 October 2009 and he served as a health care specialist. On 19 July 2013, he was honorably released from active duty to enter the Officer Training Program.
2. The DD Form 214 he was issued shows, among other awards, two awards of the AAM.
3. He provides a DA Form 638, dated 13 August 2013, which shows he was recommended for award of the AAM with Second Oak Leaf Cluster (three awards) for meritorious service during the period 18 July 2011 to 19 July 2013. The reason for the award was ETS. Item 8 (Previous Awards) of this form shows he had two previous awards of the AAM. This form also shows the recommendation was approved by the approval authority on 16 August 2013 and orders were published for the award.
DISCUSSION AND CONCLUSIONS:
The applicant's DD Form 214 shows two awards of the AAM. However, the
DA Form 638 provided by the applicant shows he was awarded the AAM with Second Oak Leaf Cluster after he was discharged. Therefore, his DD Form 214 should be corrected to show he was awarded three awards of the AAM.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the entry "ARMY ACHIEVEMENT MEDAL (2ND AWARD)" from his
DD Form 214 and replacing it with the entry "Army Achievement Medal
(3rd Award)."
