THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to list all awards and decorations as a result of his service in Iraq.
2. The applicant states he was awarded several medals, but they were not entered on his DD Form 214.
3. The applicant provides the following additional documents:
* statement of service, dated 3 February 2009
* DD Form 214, dated 2 March 2009
* orders for the Combat Medical Badge, dated 1 November 2009
CONSIDERATION OF EVIDENCE:
1. The applicant's records show he was appointed as an Army Nurse in the U.S. Army Reserve and executed an oath of office on 4 August 2003. He completed the Army Medical Department Basic Course and was promoted to captain on 8 April 2008.
2. On 5 November 2008, he was ordered to active duty in support of Operation Iraqi Freedom and subsequently served in Iraq from 15 November 2008 to 16 February 2009. He was assigned to the 945th Medical Detachment.
3. He was honorably released from active duty on 2 March 2009 to the control of his U.S. Army Reserve unit.
4. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of his DD Form 214 shows the Army Commendation Medal, Army Reserve Components Achievement Medal, National Defense Service Medal, Global War on Terrorism Service Medal, Army Service Ribbon, and Armed Forces Reserve Medal with "M" Device.
5. Headquarters, Task Force 1st Medical Brigade, Permanent Orders 305-014, dated 1 November 2009, awarded him the Combat Medical Badge for satisfactory performance of medical duties while his unit was engaged in active ground combat or being engaged by the enemy on 3 January 2009.
6. Headquarters, Medical Task Force 10, memorandum, dated 3 February 2009, authorized the applicant the Iraq Campaign Medal, Global War on Terrorism Service Medal, and overseas service bar followed by the statement, "one stripe = 6 months of combat service."
7. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of Operation Iraqi Freedom. Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-date to be determined)
8. Army Regulation 670-1 (Uniforms and Insignia) governs the requirements for the overseas service bar. A bar is authorized for wear for each period of active Federal service as a member of the U.S. Army outside of the continental limits of the United States for the specific time frames and areas of operation cited in Army Regulation 670-1 or appropriate Department of the Army message. There are special provisions regarding authorization for the overseas service bar for service in a hostile fire zone and for combining service to calculate award of the bars. For Iraq service, one overseas service bar is authorized for each period of 6 months active Federal service as a member of a U.S. Service in Iraq from 19 March 2003 to a date to be determined. The months of arrival to and departure from the area of operation are counted as whole months.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends that his DD Form 214 should be corrected to show various Iraq-related awards.
2. Permanent orders awarded him the Combat Medical Badge which is not shown on his DD Form 214; therefore, he is entitled to correction of his DD Form 214 to show this badge.
3. He served in Iraq from 15 November 2008 to 16 February 2009. Therefore, he served a qualifying period of service for award of the Iraq Campaign Medal and he is entitled to correction of his records to show this award. Additionally, he participated in the Iraqi Surge and Iraqi Sovereignty campaigns during his service in Iraq and is also entitled to award of two bronze service stars to be affixed to the Iraq Campaign Medal.
4. With respect to the overseas service bar, he served in Iraq from 15 November 2008 to 16 February 2009. As this period of service is less than 6 months, it does not entitle him to an overseas service bar. In any case, the overseas service bar is an item of clothing governed by Army Regulation 670-1 and there is no provision to list items of clothing on the DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to his DD Form 214 the Combat Medical Badge and the Iraq Campaign Medal with two bronze service stars.
2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to the one overseas service bar.
