THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 1 October 2009 to show his prior active service, awards, decorations, and any other information that should have been carried over from a previously-issued DD Form 214.
2. The applicant states he served on active duty from 1 December 2003 to 25 October 2004 and was issued a DD Form 214 for this period of service. However, this form was not available at the time of his release from active duty in 2009 and some information was left out.
3. The applicant provides the following documents:

CONSIDERATION OF EVIDENCE:
1. The applicant requests correction of his DD Form 214 for the period ending 1 October 2009 to show his prior active and prior inactive service; however, there is no indication he submitted his application to Commander, U.S. Army Human Resources Command (HRC), ATTN: AHRC-PSR, 1 Reserve Way, St. Louis, MO 63132-5200, for calculation of his active and inactive service and/or he was denied relief. He was notified by separate correspondence to submit his request to HRC-St. Louis with supporting documentation. Since he is required to exhaust all administrative remedies, there will be no further discussion of this issue in this Record of Proceedings.
2. The applicant's records show he enlisted in the U.S. Army Reserve (USAR) on 11 February 2002. He subsequently entered active duty for training (ADT) on 24 July 2002, completed basic combat and advanced individual training, and was awarded military occupational specialty 38A (Civil Affairs Specialist). He was honorably released from ADT on 16 January 2003.
3. The DD Form 214 he was issued shows he completed 5 months and 23 days of creditable active service. Additionally, item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) shows the Army Service Ribbon and Expert Marksmanship Qualification Badge with Rifle Bar.
4. On 1 December 2003, he was ordered to active duty in support of Operation Iraqi Freedom (OIF) and subsequently served in Iraq from 3 February 2004 to 10 October 2004.
5. He was honorably released from active duty on 25 October 2004 to the control of his USAR unit. The DD Form 214 he was issued shows he completed 10 months and 25 days of creditable active service during this period.

6. On 24 August 2008, he was again ordered to active duty in support of OIF and subsequently served in Iraq from 12 October 2008 to 18 August 2009. He was honorably released from active duty on 1 October 2009 to the control of his USAR unit. The DD Form 214 he was issued shows he completed 1 year, 1 month, and 8 days of creditable active service. Additionally, this form shows the following entries:

7. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of his or her military service. It is important that information entered on the form should be complete and accurate. Chapter 2 contains guidance on the preparation of the DD Form 214. Chapter 2 states that item 12 shows the record of service. Extreme care is used when completing this item since post-service benefits, final pay, retirement credit, and so forth are based on this information.
8. Army Regulation 600-8-22 (Military Awards) provides for the following awards:
a. The Global War on Terrorism Expeditionary Medal is authorized for award to members who deployed abroad for service in the Global War on Terrorism operations on or after 11 September 2001 to a date to be determined. The general area of eligibility encompasses all foreign land, water, and air spaces outside the 50 states of the United States and outside 200 nautical miles of the shores of the United States. The Secretary of Defense will designate the specific area of eligibility per qualifying operation.
b. The Iraq Campaign Medal is awarded to members who have served in direct support of OIF. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of OIF. Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-date to be determined)
DISCUSSION AND CONCLUSIONS:
1. The applicant contends his last DD Form 214 should be corrected to show his prior awards.
2. The evidence of record shows the applicant was awarded the Global War on Terrorism Expeditionary Medal during for his first mobilization to Iraq/Kuwait. However, this award is not shown on his DD Form 214 for the period ending 1 October 2009; therefore, he is entitled to correction of this form to show this award.
3. The evidence of record shows the applicant was awarded the Expert Marksmanship Qualification Badge with Rifle Bar during his ADT. However, this badge is not shown on his DD Form 214 for the period ending 1 October 2009; therefore, he is entitled to correction of this form to show this badge.
4. The evidence of record shows the applicant participated in two campaigns (Iraqi Surge and Iraqi Sovereignty) during his service in Iraq. Therefore, he is entitled to award of two bronze service stars with his already-awarded Iraq Campaign Medal and correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the Iraq Campaign Medal with campaign star from item 13 of his DD Form 214 for the period ending 1 October 2009 and
b. adding the Global War on Terrorism Expeditionary Medal, Expert Marksmanship Qualification Badge with Rifle Bar, and Iraq Campaign Medal with two bronze service stars to item 13 of his DD Form 214 for the period ending 1 October 2009.
