THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008, be removed from his Official Military Personnel File (OMPF).  

2.  The applicant states that Permanent Order 199-99 had the wrong Social Security Number (SSN) (i.e., his SSN) and was posted to his OMPF.  He also states that Permanent Order 199-99 was corrected. 

3.  The applicant provides a copy of Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008; a corrected copy of Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008; and his orders for the first award of the Good Conduct Medal in support of his application.

CONSIDERATION OF EVIDENCE:

1.  The applicant is currently serving on active duty in the rank/grade of sergeant (SGT)/E-5. 

2.  The applicant provides a copy of Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008, for award of the Good Conduct Medal for the period 12 January 2005 to 11 January 2008.  These orders show another Soldier's name and erroneously show the applicant's SSN.

3.  The applicant provides a "corrected copy" of Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008, which shows the applicant's SSN was replaced by the other Soldier's correct SSN.

4.  A review of the performance, the Military Personnel Records Jacket (MPRJ), and the Deployment/Mobilization sections of the applicantÂ’s OMPF on the interactive Personnel Electronic Records Management System (iPERMS) revealed a copy of the original Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008.

DISCUSSION AND CONCLUSIONS:

Since Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008, shows another Soldier's name, which order was subsequently corrected, it would be appropriate to remove this permanent order from the applicant's OMPF.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by removing Headquarters, 62nd Engineer Combat Battalion (Heavy) Permanent Order 199-99, dated 17 July 2008, from the applicant's OMPF/iPERMS records.
